摘　　要：探讨药学服务模式转型中我院药师在不合理处方干预、处方点评、药师参与临床治疗、药品不良反应监测、合理用药宣传和咨询等方面开展多种形式的药学技术信息服务,自觉成为医、药、护医疗团队中的一员。This article is to discuss what we pharmacists are doing in the pharmaceutical service mode transformation. We carry out various kinds of pharmaceutical technology information service as follows：adverse drug reaction monitoring and reporting, unreasonable prescription intervention, prescription reviews, education for rational drug use, medication consultation, etc., thus consciously become a member of the medical and nursing team.
