两种黄体酮阴道制剂在IVF/ICSI-ET黄体支持中的用药比较

摘　　要：目的：比较黄体酮阴道缓释凝胶（雪诺同）和黄体酮软胶囊（安琪坦）经阴道给药作为体外受精-胚胎移植（IVF-ET）/卵泡浆内单精子注射授精（ICSI）后常规黄体支持用药的效果。方法：回顾性分析河南省人民医院2012年6月~2012年12月常规长方案新鲜移植周期共591个周期,按照黄体支持使用药物的不同,随机分为雪诺同组和安琪坦组,其中雪诺同组共228个周期,安琪坦组共363个周期,比较两种药物在IVF黄体支持中的有效性。结果：两组间临床妊娠率、种植率、活产率比较,雪诺同组（61.84%,45.39%,52.63%）均高于安琪坦组（60.05%,43.29%,49.59%）;两组早期流产率、异位妊娠率比较,雪诺同组（8.51%,0）低于安琪坦组（10.09%,0.92%）,但差异均无统计学意义（P〉0.05）。结论：使用雪诺同作为IVF/ICSI-ET后黄体支持常规用药较安琪坦获得更好的临床效果,有减少早期流产的可能。Objective： To compare the efficacies of progesterone sustained-release vaginal gel and progesterone soft capsule by vaginal administration as conventional luteal phase support drugs after fertilization embryo transfer（ IVF-ET） / follicular insemination sperm injection（ ICSI）. Methods： A total of 591 fresh embryo transfer cycles of conventional long protocol in the hospital from June 2012 to December 2012 were analyzed retrospectively,then the cycles were randomly divided into progesterone sustained-release vaginal gel group（ 228 cycles） and progesterone soft capsule group（ 363 cycles）; the efficacies in the two groups were compared. Results： The clinical pregnancy rate,implantation rate and live birth rate in progesterone sustained-release vaginal gel group were 61. 84%,45. 39% and 52. 63%,respectively,which were higher than those in progesterone soft capsule group（ 60. 05%,43. 29%,49. 59%）; the early miscarriage rate and ectopic pregnancy rate in progesterone sustained-release vaginal gel group were 8. 51% and 0. 00%,respectively,which were lower than those in progesterone soft capsule group（ 10. 09% and 0. 92%）,but there was no statistically significant difference（ P 0. 05）.Conclusion： The clinical efficacy of progesterone sustained-release vaginal gel as conventional luteal phase support drug after IVF-ET /ICSI is better than that of progesterone soft capsule by vaginal administration,which can reduce early abortion.
