老年人左心衰竭急性发作期和稳定期动态心电图观察

摘　　要：目的观察老年人左心衰竭急性发作期与稳定期动态心电图变化。方法 113例中男102例,女11例。年龄61-93岁,平均（75.4±9.8）岁。左心衰竭病史1-12年,平均（4.5±3.1）年。左心衰竭急性发作期做24 h动态心电图;稳定2周后再做24 h动态心电图。结果左心衰竭急性发作期动态心电图中窦性心动过速、室上性早搏、房速、房颤、室上性心动过速、室性早搏、室性心动过速、ST-T改变均较稳定期显著增多（P〈0.05或0.01）。结论左心衰竭急性发作期可诱发多种心律失常和心电图改变,患者并发症发生率增高,因此应高度重视急性左心衰竭的心律失常,加强对心律失常的观察和处理。Objective To investigate the characteristics of dynamic electrocardiogram in the elderly patients with left heart failure during acute period and stable period. Methods 113 patients （ 102 male, 11 female） with age of 61-93 （ 75.4±9. 8） years were enrolled in our study. All patients had a history of left heart failure 1-12（4. 5±3.1 ） years. Dynamic electrocardiogram was performed during acute period and stable period. Results The occurence of sinus tachycardia, supraventricular premature beats, atrial tachycardia, atrial fibrillation, supraventricular tachycardia, ventricular premature beats, ventricular tachycardia, ST-T change, detected by 24 hour dynamic electrocardiogram during acute period were significantly higher than that during stable period （P〈0. 05 or P〈0.01 ）. Conclusions Varieties of arrhythmia and ECG change can be induced during acute left heart failure, the incidence of complications increased, so we should attach great importance to arrhythmia resulted from acute left heart failure and strengthen the observation and treatment of arrhythmia.
