鲍氏不动杆菌的临床分布与耐药性研究

摘　　要：目的研究鲍氏不动杆菌的临床分布与耐药性,为临床治疗提供参考依据。方法对2008-2012年分离出的1 976株非重复鲍氏不动杆菌进行回顾性分析,用法国生物梅里埃公司VITEK-2分析仪对菌株鉴定及药敏试验,部分采用纸片扩散法;药敏结果按CLSI2012年版判读,采用Whonet 5.5进行统计分析。结果 1 976株鲍氏不动杆菌在科室分布以ICU为主,占65.0%,其次为呼吸内科占18.7%、干部病区占8.2%;鲍氏不动杆菌对氨苄西林、头孢哌酮、头孢他啶、头孢噻肟、庆大霉素、环丙沙星、氧氟沙星的耐药率均>60.0%,对氨苄西林的耐药率最高,为89.2%。结论鲍氏不动杆菌感染多以ICU为主,对多种抗菌药物有较高的耐药率;应加强抗菌药物的合理应用,加强病区的消毒隔离和耐药性监测。OBJECTIVE To investigate the clinical distribution and drug resistance of Acinetobacter baumanii strains ,providing a reference for clinical treatments .METHODS A total of 1976 isolated strains （from 2008 -2012） of non‐duplicated A .baumannii were analyzed retrospectively .The strain identification and drug susceptibil‐ity test were carried out by Vitek2 analyzer from BioMerieux Company .Some used paper disk diffusion .The re‐sults were analyzed by WHONET5 .5 software according to CLSI standard by 2012 .RESULTS A total of 1976 strains of non‐duplicated A .baumannii were distributed mainly at ICU ,accounting for 65 .0% .The department of respiratory medicine covered 18 .7% .The cadre wards owned 8 .2% .The drug resistance rate of A .baumannii a‐gainst ampicillin ,cefoperazone ,ceftazidime ,cefotaxime ,gentamicin ,ciprofloxacin and ofloxacin was ＆gt;60% .The drug resistance rate of A .baumannii to ampicillin was the highest ,reaching 89 .2% .CONCLUSION The infection rate of A .baumannii is still high in ICU .A .baumannii has high drug resistance rate to many antibiotics .Reasona‐ble use of antibiotics should be strengthened and the disinfection ,isolation ,and monitoring of drug resistance of the endemic area should also be enhanced .
