摘 要:目的回顾性分析医院肠杆菌科细菌的分布及耐药性,为临床抗感染治疗提供依据。方法 2007-2012年临床分离菌株经生化反应、VITEK-2鉴定卡或VITEK MS系统鉴定至种,采用K-B纸片法检测药物敏感性,细菌分布和耐药性使用WHONET5.4软件进行分析。结果共分离出非重复肠杆菌科细菌11 300株,前6位依次为大肠埃希菌、肺炎克雷伯菌、奇异变形菌、阴沟肠杆菌、产酸克雷伯菌、普通变形菌,分别占46.3%、25.1%、5.4%、4.1%、3.6%、1.7%;肠杆菌科细菌对亚胺培南、美罗培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦、阿米卡星耐药率均<10.00%,对其他抗菌药物呈现不同程度耐药,甚至是多耐药。结论肠杆菌科细菌分布广泛,对临床常用抗菌药物表现出不同程度的耐药,医疗单位应当重视耐药监测与医院感染工作,以控制耐药菌的医源性感染。OBJECTIVE To analyze the distribution and drug resistance of Enterobacteriaceae isolated from our hospital to provide information for clinical anti‐infection treatment .METHODS Clinical isolates from Jan .2007 to Dec .2012 were identified by biochemical methods ,VITEK Ⅱ identification cards and VITEK MS system .Kirby‐Bauer method was used to measure the susceptibility of clinical isolates .The data of bacterial distribution and resistance were analyzed by WHONET 5 .4 software .RESULTS A total of 11300 non‐repeated enterobacterial bac‐teria were isolated from various clinical specimens . Escherichia coli (46 .3% ) was the most frequently isolated bacteria ,followed by Klebsiella pneumonia (25 .1% ) ,Proteus mirabilis (5 .4% ) ,Enterobacter cloacae(4 .1% ) , Klebsiellaoxytoca (3 .6% ) ,and Proteus vulgaris (1 .7% ) .In this study ,for enterobacterial bacteria ,drug resistance to imipenem ,meropene ,cefoperazone/sulbactam ,piperacillin/tazobactam and amikacin was less than 10% ,whereas it was found different degrees of resistance to other antimicrobial agents ,even multi‐drug resist‐ance .CONCLUSION Clinical enterobacterial isolates exhibited significant resistance to clinical common used antibi‐otics .Strong attention should be paid to the monitoring of resistant pathogens and hospital infection to control hospital acquired infection caused by resistant bacteria .
