MTP干预法对骨外科住院患者抗菌药物合理应用的影响

摘　　要：目的观察监测-培训-计划(Monitoring-Training-Planning,MTP)干预模式对骨外科住院患者抗菌药物合理使用的干预效果,探讨促进抗菌药物的合理使用的有效干预方法。方法研究资料来源于2012年3-9月骨外科住院患者的用药信息,以合理用药国际网络(WHO/INRUD)选择性用药指标调研方法(SDUIS)设置抗菌药物合理应用观察指标;于2012年6月按照MTP干预模式,对患者抗菌药物应用进行干预,每月1次,连续干预4次,比较干预前后观察指标值的变化。结果骨外科手术患者抗菌药物的使用率、平均用药时间、联合用药率、抗菌药物使用强度分别由干预前的71.32%、5.42d、8.24%和70.31下降至干预后的58.64%、2.41d、1.05%和39.27,品种选择合理率从68.04%上升至97.89%,差异均有统计学意义(P<0.05)。结论 MTP干预模式能有效提高骨外科住院患者抗菌药物使用的合理性,具有较高的临床推广价值。OBJECTIVE To investigate the effect of monitoring‐training‐planning （MTP） intervention on rational application of antibacterials of the hospitalized patients in orthopedics department so as to put forward effective interventions to promote the reasonable use of antibiotics .METHODS The study data were collected from the in‐formation of medication of the patients who were hospitalized the orthopedics department from Mar 2012 to Sep 2012 ,then the observation indicators for reasonable use of antibiotics were set based on the selective drug use in‐dex system （SDUIS） of WHO/INRUD .In Jun 2012 ,the use of antibiotics was intervened according to the MTP intervention mode ,once a month ,for 4 times of continuous intervention ;the changes of the observation indicators were observed and compared before and after the intervention .RESULTS The utilization rate of antibiotics of the orthopedics department patients decreased from 71 .32% before the intervention to 58 .64% after the intervention , the mean medication duration from 5 .42 days to 2 .41 days ,the rate of combined use of antibiotics from 8 .24% to 1 .05% ,the intensity of use of antibiotics from 70 .31 to 39 .27 ;the rate of reasonable selection of antibiotics in‐creased from 68 .04% to 97 .89% ,there was statistically significant difference （P＆lt; 0 .05） .CONCLUSION The MTP intervention mode can effectively improve the rationality of use of antibiotics of the hospitalized patients in the orthopedics department ,and it is worthy to be promoted in the hospital .
