摘  要:目的探讨经外侧裂入路显微手术治疗高血压基底节区脑出血的临床疗效及安全性。方法回顾性总结2010-07-2012-07间在我院进行经外侧裂治疗高血压基底节脑出血30例患者的相关资料。结果 30例脑出血患者的存活率为100%。术后2个月GOS水平:优8例(26.7%);良15例(50.0%);重度病残4例(13.3%);植物生存状态3例(10.0%);死亡0例。术后5个月ADL分级情况:Ⅰ级10例(33.3%);Ⅱ级12例(40.0%);Ⅲ级5例(16.7%);Ⅳ级3例(10.0%)。结论经外侧裂入路显微手术治疗高血压基底节区脑出血能有效清除血肿,减少对脑部血管、神经及组织的损伤,具有较高的临床疗效及安全性。Objective To explore the effectiveness and safety of microsurgieal treatment via lateral fissure approach for hypertensive basal cerebral hemorrhage. Methods Retrospectively analyzing 30 patients with hypertensive basal cerebral hemorrhage in the hospital from July, 2010 to July, 2012. Results The survival rate of cerebral hemorrhage was 100%. The level of GOS after 2 months was: 8 (26.7 % ) good recovery cases, 15 (50.0%)moderate disability cases, 4 ( 13.3 % )severe disability cases, 3 ( 10.0% )persistent vegetative state cases and 0 dead cases respectively. The following up after 5 months showed that 10(33.3% )cases belonged to grade Ⅰ,12(40.0% ) cases to grade Ⅱ,5 ( 16.7% ) cases to grade Ⅲ and 3 ( 10.0% ) cases to grade Ⅳ. Conclusion The method of minimally invasive operation on hypertensive cerebral hemorrhage in basal ganglia therapy can reduce the amount of bleeding during the operation and reduce the injury of brain blood vessels, nerves and tissues. And it is in good clinical effectiveness and safety.
