儿童不同类型慢性鼻窦炎治疗方法及疗效分析

摘　　要：目的探讨儿童不同类型慢性鼻窦炎不同治疗方法的临床治疗效果,为儿童慢性鼻窦炎的治疗提供参考。方法回顾性总结并分析我院于2011-01-2013-12间收治的60例不同类型慢性鼻窦炎患儿的临床病历资料,按照患儿不同类型分为慢性鼻窦炎不伴鼻息肉组、慢性鼻窦炎伴腺样体肥大组、慢性鼻窦炎伴息肉组,每组各20例,分别采用不同方法进行治疗,总结各组患儿的临床治疗效果。结果慢性鼻窦炎不伴鼻息肉组患儿治愈8例,有效9例,治疗总有效率为85.0%;慢性鼻窦炎伴腺样体肥大组患儿治愈10例,有效7例,治疗总有效率为85.0%;慢性鼻窦炎伴息肉组患儿治愈11例,有效7例,治疗总有效率为90.0%。上述3组患儿临床治疗总有效率相比,差异无统计学意义（P〉0.05）。结论不同类型慢性鼻窦炎患儿应采取不同的治疗方式,应该首选药物治疗,伴有腺样体肥大患儿可以进行腺样体切除,伴有鼻息肉的患儿可以采用内镜鼻窦手术治疗。Objective To explore the clinical effect on different types of children＇s chronic rhinosinusitis under different therapeutic methods. Methods Studying the clinical date of 60 cases of neonatal severe hyperbilirubinemia, who received the treatment from Jan, 2011 to Dec, 2013 retrospectively. Dividing the patients, based on different types, into three groups： chronic rhinosinusitis without nasal polyps, chronic rhinosinusitis with adenoid vegetation and chronic rhinosinusitis with nasal polyps, treating them with different methods of therapy, and summarizing these three groups＇ clinical effects. Results After treatment, 8 children who had chronic rhinosinusitis without nasal polyps were cured, 9 cases improved, and the overall effective rate was 85.0% ;10 children who had chronic rhinosinusitis with adenoid vegetation were cured, 7 cases improved, and the overall effective rate was 85.0% ;11 children who had chronic rhinosinusitis with nasal polyps were cured, 7 cases improved, and the overall effective rate was 90.0%. There were no significant differences in the efficacy among the three groups （P 〉 0.05）. Conclusion Children with different types of chronic rhinosinusitis should be treated in different ways, and the treatment of systemic medical should be considered as the first choice for chronic rhinosinusitis in children. Children with adenoid vegetation are basically treated with systemic medicine combined with adenoidectomy; and children with nasal polyps endoscopic are basically treated with sinus surgery combined with administration of medicine before and after the endoscopic surgery.
