斑点追踪技术对室间隔缺损修补术前及术后左心室整体收缩功能的评价

摘　　要：目的应用二维斑点追踪技术(2D-STI)的应变参数,探讨室间隔缺损(VSD)修补术前及术后左心室整体收缩功能的变化。方法收集铁煤集团总医院行室间隔修补术的单纯室间隔缺损患儿30例,同时以健康儿童30例为对照组。应用2D-STI得出左室整体纵向峰值应变(GLPS)、左室整体径向峰值应变(GRPS)、左室整体圆周峰值应变(GCPS)。测量常规二维参数,计算左房容积,并用体表面积进行标化。结果 VSD修补术前与对照组比较显示,GLPS、GRPS、GCPS显著高于对照组,差异有统计学意义(P<0.05)。VSD修补术后与术前及对照组比较显示:GLPS、GRPS、GCPS均显著降低,差异有统计学意义(P<0.05)。左室射血分数(LVEF)各组间比较差异均无统计学意义(P>0.05)。结论 VSD术后早期左室整体收缩功能降低。2D-STI的应变参数受负荷及左室收缩力的影响,对VSD修补术前及术后左室整体收缩功能的评价具有重要意义。【Objective】To explore the changes of left ventricular Globle systolic function before and after ventricular septal defect（VSD） surgery by two-dimensional speckle tracking imaging（2D-STI） strain parameter. 【Methods】Thirty simple VSD patients that would go on VSD surgery and thirty healthy children were involved in this study. It is concluded that left ventricular globle longitudinal peak strain（GLPS）, globe radial peak strain（GRPS） and globe circumferential peak strain（GCPS） application of 2D-STI. Two-dimensional parameter were measured and LA Volume（LAV） was calculated,and standardized with the body surface area.【Results】GLPS、GRPS、GCPS in the VSD group before surery were significantly higher than the control group, the difference was statistically significant（P〈0.05）. Compared with control group and the VSD group before surery, GLPS、 GRPS、GCPS in the VSD group after surery is significantly reduced, the difference was statistically significant（P〈0.05）. Left ventricular ejection fraction showed no signigicant differences in all groups（P〉0.05）.【Conclusion】Left ventricular globle systolic function reduced after the VSD surgery in the early. 2D-STI of strain parameters influenced by load and left ventricular contraction force. To evaluate left ventricular globle systolic function before and after VSD surgery in children, 2D-STI of strain parameters is of great significance.
