改良B-Lynch缝合联合欣母沛应用于宫缩乏力性产后出血的临床观察

摘　　要：目的：探讨改良B-Lynch缝合联合欣母沛（卡前列素氨丁三醇）治疗宫缩乏力性产后出血的疗效。方法：回顾性分析2009年6月-2012年12月该院收治的难治性宫缩乏力产后出血患者60例,设其中25例为改良B-Lynch缝合联合缩宫素组,35例为改良B-Lynch缝合联合欣母沛组,并对2组术中出血量、输血量,止血时间,术后24 h出血量及药物不良反应发生情况进行比较。结果：60例均抢救成功,有效止血,保留子宫及其生理功能,术后未发生严重并发症,未见明显的药物不良反应。结论：改良B-Lynch缝合联合欣母沛治疗宫缩乏力性产后出血疗效显著,值得基层医院推广。Objective： To explore the curative effect of improved B- Lynch suture combined with Hemabate（ carboprost tromethamine） in treatment of postpartum hemorrhage caused by uterine inertia. Methods： Sixty patients with postpartum hemorrhage caused by uterine inertia treated in this hospital from June 2009 to December 2012 were analyzed retrospectively,then the patients were divided into improved B- Lynch suture combined with oxytocin group（ 25 patients） and improved B- Lynch suture combined with Hemabate group（ 35 patients）; the amounts of intraoperative blood loss,the volumes of blood transfusion,the haemostasis time,24- hour amounts of blood loss after surgery and adverse reactions in the two groups were compared. Results： All the patients were rescued successfully,hemostasis was achieved,uterus and uterine function were reserved,no severe complication occurred after surgery,no significant adverse reaction was found. Conclusion： The curative effect of improved B- Lynch suture combined with Hemabate in treatment of postpartum hemorrhage caused by uterine inertia is significant,which is worthy of clinical popularization in primary hospitals.
