浅筋膜松解术与针刺治疗腰椎间盘突出症的疗效对比研究

摘　　要：目的:对比研究微针刀浅筋膜松解术和传统针刺疗法治疗腰椎间盘突出症的临床治疗效果。方法:将60例腰椎间盘突出症患者随机分为治疗组和对照组,每组各30例。治疗组给予微针刀在腰臀腿部位浅筋膜层松解治疗,对照组予毫针常规穴位针刺治疗。采用疼痛视觉模拟评分法(Visual analogue scale,VAS)和腰椎功能障碍指数(Oswestry disability index,ODI)评估2组患者治疗前和治疗后3个月的腰腿疼痛及腰椎功能情况,对比2组临床疗效。结果:所有患者均获得3个月以上随访。两组治疗后VAS和ODI评分与治疗前比较均有下降,差异有统计学意义(P<0.05)。治疗组较对照组评分下降更加明显(P<0.05)。治疗组总有效率为96.7%,优于对照组的76.7%(P<0.05)。结论:微针刀浅筋膜松解术和传统针刺疗法都能有效缓解腰椎间盘突出症患者的腰腿疼痛症状,改善腰椎功能,两者比较,微针刀浅筋膜松解术疗法优于传统针刺疗法。Objective：To compare the efficacy of superficial fasciotomy using micro-acupotomy and traditional acupuncture in the treatment of lumbar disc herniation.Methods：Sixty patients with lumbar disc herniation were recruited and randomly divided into two groups,with 30 cases for each.Patients in the treatment group were treated with superficial fasciotomy in the waist,hip and leg.The traditional acupuncture was used in the control group.Visual analogue scale（VAS）and Oswestry disability index（ODI）were used to evaluate the efficacy of two approaches in 3-month follow-up.Results：All patients were followed up for at least 3months.The VAS and ODI scores were decreased in both groups after treatment,and the difference was statistically significant（all P〈0.05）.Moreover,patients treated with superficial fasciotomy had better outcome as compared to those treated with traditional acupuncture（P〈0.05）.In terms of efficacy,the total effective rate was96.7%in the treatment group and 76.7%in the control group（P〈0.05）.Conclusion：While both superficial fasciotomy and traditional acupuncture therapy can effectively relieve the back and leg pain and improve the lumbar vertebrae function in patients with lumbar disc herniation,superficial fasciotomy has better clinical outcome as compared to traditional acupuncture therapy.
