摘 要:目的探讨不同糖耐量者甲状腺激素水平与胰岛素抵抗及胰岛功能的关系。方法采用化学发光法检测110例2型糖尿病患者(A组)、200例糖调节异常患者(B组)和124例糖耐量正常者(C组)的空腹胰岛素(FIns)、促甲状腺激素(TSH)、游离三碘甲腺原氨酸(FT3)和游离甲状腺素(FT4)水平。结果 A组和B组FT3为(4.29±0.37)pmol/L和(4.38±0.52)pmol/L,低于C组的(4.53±0.60)pmol/L(P<0.01);A组、B组TSH水平为(2.74±1.10)IU/ml和(2.41±1.00)IU/ml,高于C组的(2.30±0.91)IU/ml(P<0.05)。FT3水平与胰岛素抵抗指数(HOMA-IR)呈负相关(P<0.05)。结论 FT3水平与胰岛素抵抗密切相关,可能参与2型糖尿病的发生发展。Objective To explore the relationship of thyroid hormones and insulin resistance and β cell function in people with different glucose tolerance status. Methods Clinical, biochemical parameters and thyroid-related hormones were measured in 110 subjects with type 2 diabetes (group A) ,200 subjects with impaired glucose metabolism(group B) and 124 subjects with normal glucose metabolism(group C). Insulin sensitivity was evaluated by HOMA-IR and β cell function evaluated by HOMA-β. Results Serum free triiodothyronine(FT3) was significantly lower in groups of A and B than that in group C[(4. 29±0.37) pmol/L and (4. 38±0. 52) pmol/L vs. (4. 53±0. 60) pmol/L] (P〈0. 05). Serum thyroid stimulating hormone(TSH) was significantly higher in groups of A and B than that in grou C[(2. 74± 1.1) IU/ml and (2.41± 1.0) IU/ml vs. (2.30±0. 91) IU/ml] (P〈0. 05). Serum level of FT3 was negatively correlated with HOMA-IR (P〈0. 05). Conclusion FT3 is significantly associated with insulin resistance and may participate in the occurrence and development of type 2 diabetes mellitus.
