地佐辛与芬太尼配伍罗哌卡因用于硬膜外分娩镇痛的比较

摘　　要：目的探讨地佐辛用于硬膜外分娩镇痛的可行性及与芬太尼比较镇痛效果以及不良反应发生率。方法选择ASAⅠ～Ⅱ级足月孕妇60例,随机分为3组,A组：地佐辛5.0 mg镇痛组,泵内以0.1%罗哌卡因（75 mg）与地佐辛5.0 mg加生理盐水配成混合液75 ml;B组：地佐辛7.5 mg镇痛组,泵内以0.1%罗哌卡因（75 mg）与地佐辛7.5 mg加生理盐水配成混合液75 ml;C组：芬太尼镇痛组,泵内以0.1%罗哌卡因（75 mg）与芬太尼0.15 mg加生理盐水配成混合液75 ml。观察硬膜外给药后镇痛及镇静评分、总产程及缩宫素用量、新生儿Apgar评分、最终分娩方式、不良反应。结果 3组间分娩镇痛效果无统计学差异（P〉0.05）;B组1分者为0,显著少于C组（P〈0.05）;3组间总产程、缩宫素用量、最终分娩方式、新生儿Apgar评分无统计学差异。结论不同剂量地佐辛、芬太尼均可用于硬膜外分娩镇痛,3组镇痛和镇静效果相似。Objective To explore the feasibility of dezocine for epidural labor analgesia and compare the analgesic effect of dezocine with that of fentanyl as well as the rate of adverse reaction. Methods Total 60 ASA Ⅰ- Ⅱ full-term pregnant women were selected and were divided randomly into three groups. Group A： dezocine 5.0 mg analgesia group, with mixture of 75 ml made up of 0.1% ropivacaine（75 mg）and dezocine（5.0 rag）plus physiological saline injected;group B：dezocine 7.5 mg analgesia group, with mixture of 75 ml made up of 0. 1% ropivacaine（75 rag）and dezocine（7.5 mg）plus physiological saline injected;group C：fentanyl group,with mixture of 75 ml made up of 0.1% ropivacaine（75 mg） and fentanyl（0.15 rag）plus physiological saline injected. And then, the epidural analgesia and sedation scores after the administration, total oxytocin dosage during the labor, neonatal Apgar score, the final delivery mode, and adverse reaction were observed. Results There was no significant difference among the three groups in labor analgesia （ P 〉 0. 05 ） ; the person number scoring one point in group B was zero, and this was significantly less than that in group C （ P 〈0. 05 ）;among the three groups, the total labor time, oxytocin dosage, the final delivery mode, and neonatal Apgar score had no significant difference. Conclusion Both dezocine of different doses and fentanyl can be used for epidural labor analgesia;the analgesic and sedative effects were similar among three groups.
