InterTan静力固定对骨质疏松股骨转子间骨折的近期疗效

摘　　要：目的探讨Inter Tan静力固定对老年骨质疏松股骨转子间骨折的近期疗效。方法回顾分析2011年7月～2013年4月,采用闭合复位Inter Tan静力固定治疗87例老年骨质疏松股骨转子间骨折患者的临床资料。男51例,女36例;年龄60～89岁,平均69.5岁。致伤原因：跌倒59例,交通事故28例;受伤至手术时间为1～20 d,中位时间6 d;根据IVANS分型：Ⅰa型7例,Ⅰb型18例,Ⅰc型27例,Ⅰd型26例,Ⅱ型9例。结果术后患者伤口均Ⅰ期愈合。48 h后鼓励护理保护下下地负重及扶拐行走。所有病例均获随访,随访时间12~24个月,平均15.6个月。1例术后发现坐骨神经损伤,术后7个月复查基本恢复。无血管损伤及深静脉血栓并发症,无内固定松动、切割及骨折不愈合等发生。术后6个月复查,根据Harris评分：优40例,良38例,可9例,优良率89.7%。结论对于老年骨质疏松股骨转子间骨折的患者,Inter Tan静力固定具有良好的近期稳定性作用,支持患者早期功能锻炼,减少卧床及相应并发症。Objective To explore the short-term curative effect of InterTan static fixation on osteoporotic intertroehanteric fracture of senile people. Methods A retrospective analysis was made for the clinical data on application of InterTan static fixation to closed restoration of osteoporotic intertrochanterie fracture of senile patients （n =87,51 males and 36 females;60-89 years old with an average age of 69.5 ）, who received treatment in our hospital from July of 2011 to April of 2013. Causes to the fracture： fall in 59 cases and traffic accident in 28 cases. The time from the injury to operation： 1-20 d; median time： 6 d. IVANS classification ： type Ⅰ a （ 7 eases） ,type Ⅰ b （ 18 cases） ,type Ⅰ c （27 cases） ,type Ⅰ d （26 eases） ,and type Ⅱ（9 cases）. Results The postoperative wound of the patients healed well in period Ⅰ After 48 hours since the operation, the patients were encouraged to walk with a crutch and even burden with the help of nurse. All the patients were followed up in 12-24 months after the operation, 15.6 months averagely. After the operation, sciatic nerve injury occurred in one case,but it was recovered basically in seven months;no complications of vascular injury and deep vein thrombosis, or internal fixation loosening, cutting or nonunion occurred. Reexamination was made six months after the operation and Harris scores were marked：excellent in 40 cases, good in 38 cases, and passable in 9 cases, with an excellent and good rate of 89.7%. Conclusion InterTan static fixation has favorable stabilizing effect on osteoporotic intertroehanteric fracture of senile people; it supports early functional exercise of patients, who need not lie on bed all the time, so as to reduce corresponding complications.
