血清降钙素原在老年下呼吸道感染中临床意义的研究

摘　　要：目的:探讨血清降钙素原在老年下呼吸道感染诊断评估的临床意义。方法:选择60例下呼吸道感染的老年患者作为感染组以及同期健康体检老年人60例为对照组。对两组入院后第1天的PCT、CRP、WBC、N的检测进行比较;评价上述4个血清指标诊断老年下呼吸道感染的灵敏度,并将采取APACHEⅡ评分机制对感染组治疗第1、8天进行评分。结果:(1)两组第1天的血清指标比较差异均有统计学意义(P<0.01);(2)血清PCT、CRP、WBC、N诊断老年下呼吸道感染的灵敏度和特异度:PCT的灵敏度最高,WBC的特异度最高。(3)感染组第1、8天的PCT值与APACHEⅡ评分值相关分析显示PCT与APACHEⅡ间具有明显正相关性(r=0.902,P<0.05)。结论:有较高敏感性和特异性的PCT可作为老年下呼吸道感染的常规检测指标,监测动态的血清PCT水平可对患者的病情进行全面的评估。Objective：To explore clinical significance in diagnosis and evaluation of serum calcitonin original in the older lower respiratory infection.Method：60 elderly patients with lower respiratory infection were selected as the infection group and another 60 healthy check-up cases were chosed as the control group. PCT, CRP, WBC, N were detected in 1 day of the two groups. The sensitivity of the above four serum index in the diagnosis of senile lower respiratory infection were evaluated, and the APACHE Ⅱ scoring mechanism was used for detecting in 1, 8 day of the infection group.Result：（1）Serum index in 1 day between the two groups had statistical significance（P〈0.01）;（2）Serum PCT, CRP, WBC, N diagnostic sensitivity and specific degrees of the older lower respiratory infection： PCT was the highest sensitivity, and the WBC specific degree was the highest;（3）The infection group in 1, 8 day of the PCT values and APACHE score values showed positive correlation（r=0.902, P〈0.05）.Conclusion：PCT has high sensitivity and specificity on diagnosis of senile lower respiratory infection, it can be used for routine detection index of senile lower respiratory infections, dynamic monitoring serum PCT level is beneficial for more comprehensive assessment to patients.
