糖皮质激素联合BCG-PSN治疗小儿支气管哮喘的Meta分析

摘　　要：目的:评价糖皮质激素联合卡介苗多糖核酸治疗小儿支气管哮喘的有效性。方法:计算机检索Cochrane Library(2012年10期),PubMed(1966-2012.10),EMBASE(1974-2012.5),万方数据库(1992-2012.10),中国期刊全文数据库(1994-2012.10),中国生物医学文献数据库(1978-2012.10),纳入卡介苗多糖核酸治疗小儿支气管哮喘的随机对照研究,采取修订后的Jadad评分量表对纳入研究进行质量评价。用Review Manage 5.1软件进行Meta分析。结果:共纳入7个研究(577位患者)。Meta分析结果显示,激素联合卡介苗多糖核酸注射剂治疗小儿支气管哮喘的总有效率,治疗前后总IgE值变化,治疗前后FEV1%值变化优于传统激素治疗,治疗前后γ-干扰素值降低幅度与传统激素治疗有明显差异,均具有统计学意义(P<0.05)。结论:糖皮质激素联合卡介苗多糖核酸治疗小儿支气管哮喘比单纯使用糖皮质激素治疗效果更佳。[ Objective] To evaluate the efficacy and safety of the treatment of bronchial asthma with hormone combined with BCG- PSN in children. [Method] We searched the electronic database of Cochrane Library （2012 - 10）, PubMed （ 1966 - 2012.10）, EMBASE （1974 - 2012.5）, the Chinese WanFang Literature Database （1992- 2012.10）, China Academic Journal （ 1994 - 2012.10）, and the Chinese Biomedical Literature（ 1978 - 2012.10）, for all the randomized controlled trials evaluating the efficacy and the safety of bronchial asthma with BCG- PSN in children. Meta-analysis was carried out with Review Manager 5.1 software and the Revised Jaded scale was used to access the quality of these studies. [Result]The efficiency of to- tal conventional hormone therapy and hormone combined with BCG polysaccharide nucleic acid injection in trealment of children with bronchial asthma and the changes of total IgE value before and after treatment, changes of FEV1% value is better than simple hormone therapy group. There was significant difference between the three treamts in decreasing amplitude of interferon gamma value before and after treatment. [ Conclusion] Traditional glucocorticoid inhalation combined with BCG - PSN injection is better than using glucocorticoid inhalation.
