ABO型新生儿溶血症筛查结果分析

摘　　要：目的:了解夫妇ABO血型不合的孕妇血清中IgG抗-A(B)效价异常率及临床意义。方法:采用回顾性分析方法分析ABO-新生儿溶血病(HDN)患儿的胆红素水平与新生儿黄疸患儿的差异,并追溯其母亲年龄、产检时血型血清抗体效价及父亲血型,分析其与发病率之间关系。结果:ABO-HDN导致的新生儿黄疸与其他原因导致的新生儿黄疸患儿总胆红素和间接胆红素水平间差异无统计学意义(P>0.05);74例患儿母亲血型血清抗体效价中有40人异常,占总人数的54.05%;比较父亲血型,A、B型血型的父亲HDN发病率高于AB型(29%∶32%∶13%),但AB血型的父亲其妻子抗体效价更高(61.54%∶53.12%∶51.72%)。结论:在产前保健孕妇中进行ABO血型抗体效价检测,及早发现异常及时治疗,可减少由于母婴血型不合引起的HDN的发生。Objective：To investigate serum anti-IgGA（B） antibody abnormal titer rate and the clinical signifi cance of the ABO blood group incompatibility pregnant women. Method：Using retrospective analysis methods to analyze the difference of bilirubin level between the ABO blood group incompatibility caused hemolytic disease of the newborn （ABO-HDN） and neonatal jaundice patients, then trace the mother＇s age, the serum antibody titer and father＇s blood group and then analyze the relationship about the incidence of ABO-HDN. Result：There were no significant differences of total bilirubin and indirect bilirubin level between ABO HDN and neonatal jaundice in- duced by other reasons （P〉0.05）. Forty mother＇s blood group serum antibody titer were abnormal in seventy-five cases, accounting for 54.05% of the total; compared with the father＇s blood group, the ABO-HDN incidence rate was higher in A-blood type and B-blood group fathers than AB-blood group fathers（29% vs 32% vs 13%）, and the wives of Aid blood group fathers had the higher antibody titer （61.54% vs 53.12 vs 51.72%）. Conclusion： Detected the AB O-blood group antibody titer in pregnant women and found the abnormal situations and obtained treatment in early time could decrease the incidence of ABO-HDN.
