摘 要:目的探讨冠状动脉造影术前超声测量患者主动脉根部内径对术中X线曝光时间、曝光剂量及造影剂用量的影响。方法行冠状动脉造影术患者498例,随机分为观察组261例和对照组237例,观察组冠状动脉造影术前行超声检查,测量主动脉窦内径和升主动脉内径,计算主动脉管壁距对侧冠状动脉间的距离,并根据其结果选择合适造影导管;对照组直接选择5F多功能造影导管。比较2组X线曝光时间、曝光剂量、造影剂用量及更换导管次数。结果观察组X线曝光时间((15.7±6.3)min)、曝光剂量((4 132.7±29.4)mGy)、造影剂用量((80.2±19.5)mL)、更换导管次数(19次)均低于对照组((23.5±4.8)min、(7 365.3±37.9)mGy、(130.1±17.7)mL、56次),2组比较差异有统计学意义(P<0.05)。结论冠状动脉造影术前超声测量患者主动脉根部内径,有助于冠状动脉造影导管的准确选择,减少更换导管次数,缩短X线曝光时间,减少X线辐射量及造影剂用量。Objective To investigate the influence of preoperative measurement of aortic root diameter by echocardiography on the X-ray exposure time,exposure dose and dose of contrast agent in coronary angiography(CAG).Methods A total of 498 patients undergoing CAG were randomly divided into observation group(n=261)and control group(n=237).The parameters such as the diameter of the aorta,ascending aorta and the distance between the aorta and the contralateral coronary artery(A-AD)were measured by echocardiography before CAG in observation group.The angiography catheter was chosen according the A-AD.5F multifunctional angiography catheter was selected directly in control group.The X-ray exposure time,exposure dose,dose of contrast agents and changing times of angiography catheter were compared between two groups.Results The X-ray exposure time,exposure dose,dose of contrast agents and changing times of angiography catheter were(15.7±6.3)min,(4 132.7±29.4)mGy,(80.2±19.5)mL and 19 in observation group,significantly lower than those in control group((23.5±4.8)min,(7 365.3±37.9)mGy,(130.1±17.7)mL,56)(P〈 0.05).Conclusion The catheter selection according to aortic root diameter assessed by echocardiography before CAG procedure,may contribute to choosing suitable angiography catheter,shortening the X-ray exposure time and dose,and reducing the changing times of catheter and the dose of contrast agents in CAG procedure.
