摘　　要：目的探讨采用自体脂肪移植矫正凹陷性瘢痕的效果与注射移植技巧。方法自2009年7月至2014年3月，共接诊21例（24处）凹陷性瘢痕患者。移植前1d以生理盐水注射法评估注射量。用注射针头往返穿刺松解受区瘢痕粘连，然后行多层面、多隧道自体脂肪注射移植。结果21例（24处）患者注射盐酸肾上腺素生理盐水5～40ml，自体脂肪注射移植量为6～48ml，注射后凹陷畸形得到矫正，局部稍隆起；1个月后有部分吸收，隆起消退，注射处与周围皮面相平；6个月后，局部基本平坦，患者均满意。所有患者均未出现硬结、囊肿、感染等并发症。结论只要患者病情适合，注射方法得当，自体脂肪移植是矫正凹陷性瘢痕的良好方法。Objective To study the curative effect and injective technique of autologous fat transplantation for depressed scar. Methods From July 2009 to March 2014, 21 patients with 24 depressed scars were treated. The day before transplantation, normal saline injection method was used to evaluate injective quantity of autologous fat, cicatricial adhesion of acceptor site was loosened by the injective needle puncture repeatedly and then the autologous fat was transplanted by multi-layers and multi-tunnels. Results About 5 to 40 ml nor- mal saline plus adrenalin hydrochloride was injected in 24 depressed scars from 21 patients respectively. The transplanted quantities of autologous fat were from 6 to 48 ml, and after injective transplantation, the depressed deformities were corrected totally with local slightly swelling which disappeared at one month and were in the same level with surrounding. The patients felt satisfactory at 6 months without any complications, such as scleroma, cyst and infection. Conclusion Under the condition of suitable case selection and correct transplantation method, autologous fat graft might be a good method to correct depressed scar.
