摘　　要：目的探讨改良版患者自评主观整体营养评估量表(abPG-SGA)在消化道肿瘤化疗患者营养筛查中的适用性。方法采用方便抽样法抽取150例消化道肿瘤化疗患者,在患者入院24h内采用患者自评主观整体营养评估量表(PG-SGA)、abPA-SGA、营养风险筛查2002(NRS 2002)进行营养筛查。通过受试者工作特征曲线(ROC)测量筛查工具的灵敏度、特异度等,通过Kappa值分析一致性。结果 150例患者均完成营养筛查,PG-SGA测得患者营养不良发生率为54.00%。与金标准比较,abPG-SGA与NRS2002的灵敏度、特异度、阳性预测值、阴性预测值、Youden指数和Kappa值分别为0.926、0.986、0.987、0.919、0.911、0.907vs0.852、0.783、0.826、0.844、0.635、0.663。结论在消化道肿瘤化疗患者中,abPG-SGA的灵敏度、特异度、阳性预测值、阴性预测值等均高于NRS 2002,与PG-SGA一致性较好,且简单易行,适用于消化道肿瘤化疗患者的营养筛查。Objective To explore the feasibility of abridged scored Patient-generated Subjective Global Assessment （abPG-SGA） in digestive tract cancer patients receiving chemotherapy. Methods A total of 150 digestive tract cancer patients receiving chemotherapy were selected by convenience sampling method. Within 24 hours after their admission, they were assessed using PG-SGA, abPG- SGA and Nutritional Risk Screening 2002 （NRS 2002）. The specificity, Youden＇s index of the screening tools were calculated by using ROC curves positive predictive value, negative predictive The consistency between tools was analyzed by Kappa value. Results The nutrition screening of the 150 patients had been assessed and the prevalence of malnutrition was 54.00% by PG-SGA. Compared with gold standard, the sensitivity, specificity, positive predictive value, negative predictive va lue, Youden＇s index and Kapppa value of abPG SGA were 0. 926,0. 986,0. 987,0. 919, 0. 911 and 0. 907, whereas these values of NRS 2002 were 0. 852,0. 783,0. 826,0. 844, 0. 635 and 0. 663 respectively. Conclusion The sensitivity, specificity, positive predic- tive value and negative predictive value of abPG-SGA were higher than those of NRS 2002. It was well consistent with PG-SGA and easy to perform. Therefore, the abPGSGA was more suitable for nutrition screening in clinical practice.
