护士PICC技术准入制对患者PICC并发症的影响

摘　　要：目的探讨护士PICC技术准入制对患者PICC并发症的影响,旨在预防和减少患者PICC并发症.方法：采用对照试验.设定试验组：收集自实行护士PICC技术准入制以来全院范围内接受PICC置管住院病例.对照组：回顾调查实行护士PICC技术准入制之前全院范围内接受PICC置管住院病例.试验组在实行PICC技术准入制的基础上,由具有PICC技术准入资质的护士参与完成PICC技术临床应用.记录两组患者保留PICC导管时间.观察患者有无PICC并发症,如穿刺部位血肿、渗血、导管异位、心律失常、穿刺点感染、静脉炎、导管堵塞、导管移位和导管非计划性脱落等.结果：实行护士PICC技术准入制以后患者PICC并发症发生率为10.7%,实行护士PICC技术准入制以前患者PICC并发症发生率为26.0%,实行护士PICC技术准入制以后患者PICC并发症发生率与实行护士PICC技术准入制以前比较差异有统计学意义（X^2=50.28,P〈0.01）.结论：实行护士PICC技术准入制可减少患者PICC并发症的发生,护士PICC技术准入制值得推广应用.ObjectiveInvestigating the effect of the qualification certification system of peripherally inserted central catheter （PICC） nurse on complications caused by retainedPICC, to prevent and reduce the complications. Method ： A controlled clinical trial was carried out. All the inpatients retained PICC after carrying out the qualification certificationsystem of PICC were as the test group. And the ones before carrying out it were as the control group. The test group was trained by the nurses having the qualification certification ofPICC. Observe the catheter retention time and complications such as local hematoma at the puncture site、errhysis、catheter dystopy、arrhythmia、infection of puncturation、Phlebitis、catheter obstruction、non-planned decannulation and so on.Results ：The incidence of complications caused by retained PICC after carrying out the qualification certification systemof PICC was 10.7%, while it was 26.0% before carrying out the system. There was significant difference between the two groups（X^2=50.28,P〈0.01）.Conclusions ：It can reduce theincidence of complications caused by the PICC by carrying out the qualification certification system of PICC, and it is to be worth spread.
