腋窝淋巴结阳性乳腺癌患者接受新辅助化疗后行前哨淋巴结活检可行性的Meta分析

摘　　要：目的:系统评价前哨淋巴结活检用于淋巴结阳性乳腺癌患者接受新辅助化疗后(NAC)的可行性。方法:计算机检索Pubmed、Cochrane Library、Springer、Elsevier、National Technical、万方等期刊全文数据库,纳入新辅化疗后乳腺癌患者行前哨淋巴结活检的研究文献,检索时间2000年1月至2013年12月,对符合纳入标准的文献进行Meta分析。结果:共纳入10篇临床研究,共纳入患者1 437名。合并前哨淋巴结活检敏感度为0.867(95%可信区间:0.835-0.894),特异度为1.000(95%可信区间:0.988-1.000)。结论:腋窝淋巴结阳性乳腺癌患者接受新辅助化疗后行前哨淋巴结活检诊断敏感性和特异性较高,可行性好,准确率高。Objective：To evaluate the utility of the sentinel lymph biopsy in patients with breast cancer and positive axillary nodes treated with neoadjuvant chemotherapy.Methods：Literature search was performed by using Pubmed,Cochrane Library,Springer,Elsevier,National Technical and Wangfang data from January 2000 to December 2013 for research the sentinel lymph node biopsy after neoadjuvant chemotherapy in patient with breast cancer.The study which met the quality standard was enrolled for meta analysis.Results：There were 10 studies in which,a total of 1 437 patients involved in Meta-analysis.The pool sensitivity and specificity were found to be 0.867（95%CI：0.835-0.894）and 1.000（95%CI：0.988-1.000）respectively.Conclusion：For patients with breast cancer and positive axillary nodes,sentinel lymph node biopsy after neoadjuvant chemotherapy have a good feasibility and high accuracy,and the sensitivity and specificity are relatively high.
