甲型乙型肝炎联合疫苗免疫后的甲肝免疫效果观察

摘　　要：目的评价儿童不同剂次甲型乙型肝炎联合疫苗免疫后甲型肝炎的免疫效果。方法随机选择玉环县户籍5～14岁儿童，其中326名儿童免疫1剂次甲型乙型肝炎联合疫苗，228名儿童免疫3剂次甲型乙型肝炎联合疫苗。分别于免疫前、1剂组免后6个月、3剂组第3针免后1个月采集静脉血，分离血清后检测甲肝抗体。结果儿童免疫甲型乙型肝炎联合疫苗后甲肝抗体阳性率为100．00％。甲肝抗体阳性对象中免疫1剂组与免疫3剂组甲肝抗体IgG浓度中位数免疫前分别为8．330、7．130mlU／mL，免疫后分别为13．410、14．640mlU／mL，两组免疫前后比较差异均有统计学意义（P〈0．叭）；甲肝抗体阴性对象中免疫l剂组与3剂组甲肝抗体IgG浓度中位数免疫前分别为0．380、0．330mlU／mL；免疫后分别为8．700、13．550mlU／mL，两组免疫后甲肝抗体IgG水平均分别高于免疫前，差异均有统计学意义（P〈0．01）。结论5岁以上儿童免疫1剂或3剂甲型乙型肝炎联合疫苗后，1个月和6个月均显示有效的免疫反应。Objective To evaluate immunogenicity efficacy with hepatitis A and B combined vaccines in different doses among children. Methods Local children aged five to fourteen in Yuhuan County were selected randomly. Among all the participants, 326 children received one dose of hepatitis A and B combined vaccine immunization and 228 children received three doses of hepatitis A and B combined vaccine immunization. Serum samples collected before and one or six months after the full inoculation, HAV - IgG were tested. Results The positive antibody rate was 100. 00% ＇after inoculation. Among the children with HAV - IgG positive, the median concentration of HAV - IgG before inoculation were 8. 330 mIU/ mL and 7. 130 mIU/mL respectively, significantly lower than that after inoculation with 13. 410 ndU/mL and 14. 640 mIU/ mL respectively （ P 〈 0. 01 ）. Among the children with HAV - IgG negative, the median concentration of HAV - lgG befere inoculation were 0. 380 mlU/mL and 0. 330 mIU/mL respectively, significantly lower than that after inoculation with 8. 700 mIU/mL and 13. 550 mIU/mL respeetively（P 〈 0. 01 ）. Conclusion Positive effect can be observed with both one and three doses of hepatitis A and B combined vaccine among the children aged above 5 years old.
