摘　　要：目的：翻译、跨文化修订形成机构版慢性病保健评估问卷（ assessment of chronic illness care，ACIC）简体中文版，并评价其信效度。方法引进 ACIC，进行翻译、回译及文化调试，最终修订为包含七个维度，34个条目的 ACIC 简体中文版。并于2013年11月-2014年2月，采用多阶段分层整群随机抽样法在湖南省抽取102家基层医疗卫生机构的400名医务人员进行问卷调查，对问卷进行内部一致性、重测信度、内容效度、因子分析等信效度分析。结果 ACIC 简体中文版可行性良好（得到有效问卷390份，有效回收率为89.0%）。问卷的 Cronbach＇s α系数为0.826，问卷各维度的 Cronbach＇s α系数为0.880~0.930；重测信度为0.722（P 〈0.01）；七因子结构方程模型拟合良好，χ2/ df 为2.70，RMSEA 为0.066。结论 ACIC 简体中文版有较好的信度、效度，可作为特异性 ACIC，用于评价医疗机构慢性病保健服务能力及水平。Objective To design the simplified Chinese version of Assessment of Chronic Illness Care（ ACIC）in mechanism edition by translation and cross - cultural revision and evaluate its reliability and validity. Methods We introduced ACIC from abroad and revised it into simplified Chinese edition including 7 dimensions and 34 items by translation,back transla-tion and culture adjustment. Using multi - stage stratified cluster random sampling method,from Nov,2013 to Feb,2014,400 personnel selected from 102 community health service institutions in Hunan province were investigated with questionnaire about the internal consistency,the test - retest reliability,content validity and factor analysis. Results The simplified Chinese version of ACIC had good feasibility（returned valid questionnaires were 390 and the valid recovery rate was 89. 0% ）. The Cronbach＆#39;s αcoefficients was 0. 826 and that of all factors of ACIC ranged from 0. 880 to 0. 930. Test - retest reliability coefficients was 0. 722 （P ＆lt; 0. 01）. Structural equation model showed good fit,χ2 / df being 2. 70 and RMSEA 0. 066. Conclusion The Chinese ver-sion of ACIC has good reliability and validity,which can be used as a kind of specific ACIC to evaluate medical institutions＇ serv-ice ability and level in treating chronic diseases.
