摘  要:目的:通过针刺大鼠心经神门与少海两穴,观察其对补阳还五汤成分抗脑缺血作用及代谢影响,验证"穴药"法能研究中药归经及经络脏象的可行性;方法:采用改良栓线法制备大鼠脑缺血模型;针刺大鼠心经的神门与少海;观察补阳还五汤的药效指标及代谢指纹图谱的变化。采用高效液湘法建立血液指纹图谱并用相似度评价法进行评价。结果:与对照组比较,针刺药物组与药物组的谷氨酸(GLU)、白细胞介素-6(IL-6)、乳酸脱氢酶(LDH)、血管内皮细胞生长因子(VEGF)、γ-氨基丁酸(γ-GABA)、转化生长因子-β1(TGF-β1)、热休克蛋白70(HSP70)、一氧化氮(NO)、肿瘤坏死因子-α(TNF-α)均产生显著性的变化,其中VEGF、TGF-β1上调,IL-6、LDH、GLU、γ-GABA、HSP-70、NO、TNF-α下调,说明针刺药物组及药物组能使缺血性脑组织血管新生、兴奋性氨基酸的毒性、神经损害、氧自由基损伤、炎症反应等降低,神经功能改善,其中针刺药物组更为明显;针刺药物组内、药物组内、对照组内、针刺药物与药物组间、针刺药物与对照间组、药物与对照组间的补阳还五汤指纹图谱或血液指纹图谱的相似度为0.1711-0.6041,各组间、组内差异显著(P<0.01),针刺能明显改变补阳还五汤代谢,促进其向对照组方向代谢。结论:针刺经络腧穴能调整脏腑功能,改变药物对其作用与代谢规律,"腧穴针刺作用-经络脏腑功能-药物作用与代谢"存在一定的印迹规律,据此可建立中药归经与经络脏象研究的"穴药"法。Objective: Study the effects of acupuncture on Shenmen (HT 7) and Shaohai (HT 2) points in rat heart meridian of anti-cerebral ischemia and constituent metabolite by Buyang Huanwu Decoction to verify the feasibility of the ‘point-drug' method on the study of channel tropism of traditional Chinese medicine, meridian and viscera. Methods: The cerebral ischemia reperfusion rat model was established with the middle cerebral artery occlusion (MCAO) method. Shenmen (HT 7) and Shaohai (HT 2) points in heart meridian were selected. The effect of anti-cerebral ischemia and metabolic fingerprint of the Buyang Huanwu Decoction were observed. The blood fingerprints were established with the high performance liquid chromatography (HPLC), which were evaluated by the similarity. Results: In comparison with the control group, there were obvious changes on the expressions of IL-6, GLU, LDH, VEGF, TGF- β1, γ-GABA, HSP-70, NO and TNF- α in both acupuncture combined with drug group and the drug group, in which the expressions of VEGF and TGF- β1 were up-regulated whereas the expressions of IL-6, LDH, GLU, γ-GABA, HSP-70, NO and TNF-α were down-regulated. It was suggested that acupuncture combined with drug group and the drug group might promote the angiogenesis and improve neurological function that proven by reducing the excitatory amino acid, nervous lesion, oxygen free radical injury and inflammatory reaction, especially in the acupuncture combined with drug group. The similarity range of Buyang Huanwu Decoction fingerprint or blood fingerprint was 0.6041-0.1711 among the acupuncture combined with drug intra-group, the drug intra-group, the control intra-group, between the acupuncture combined with drug intra-group and the drug group, between the acupuncture combined with drug intra-group and the control group, and between the drug group and the control group. There were significant differences among each group and each intra-group. It turned out that acupuncture could obviously change the
