参附注射液治疗前循环心功能不全性分水岭脑梗死的临床研究

摘　　要：目的:评价参附注射液治疗前循环心功能不全(CD)性分水岭脑梗死(CWI)的效果。方法:将2008年1月-2013年1月经头颅磁共振弥散成像证实存在前循环CWI及临床存在CD的住院患者63例随机分为参附(SF)组32例和丹参(DS)组31例进行对照研究。比较入院及出院时患者的心、脑功能,经颅多普勒及血清学参数,分析SF的疗效。结果:组内比较,两组治疗后临床各参数均较治疗前有改善(P<0.01)。组间比较,入院时两组间各临床参数无统计学差异;治疗后与DS组相比较,SF组临床CD评分、美国国家卫生院脑卒中量表(NIHSS)和改良的Rankin评分(m RS)及血清B型钠尿肽和纤维蛋白原水平、血红细胞比容明显下降(P<0.05,P<0.01),而心脏射血分数,大脑中、大脑前及颈动脉的平均血流速度和痊愈率水平明显上升(P<0.05,P<0.01)。结论:参附注射液能有效改善患者的CD并缓解前循环CWI症状。Objective： To evaluate the effect of Shenfu Injection on cerebral watershed infarction （CWI） in anterior circulation induced by cardiac dysfunction （CD）. Methods： From January 2008 to January 2013, 63 inpatients with CD, suffering from CWl in anterior circulation proved by head diffusion weighted imaging, were randomly divided into Shenfu （SF） group and Danshen （DS） group. The cardiac function and brain function were determined before and after treatment. The effect of Shenfu Injection was evaluated with transcranial doppler echocardiography and serologic parameters. Results： According to the comparison within groups, the clinical parameters after the treatment in both groups were better than those before treatment, respectively （P〈0.01）. On the basis of comparison between groups, there was no significant difference in the clinical parameters between the two groups at the admitted time. But, after the treatment, compared with those in DS group, the scores of clinical CD, national institutes of health stroke scale （NIHSS） and modified Rankin scale （mRS）, the levels of B type natriuretic peptide and fibrinogen in serum in SF group significantly decreased （P〈0.05, P〈0.01）, while the cardiac ejection fraction, the mean velocities of middle cerebral artery, anterior cerebral artery and carotid artery and the hematocrit value in serum signifcantly increased （P〈0.05, P〈0.01）. Conclusion： Shenfu Injection could effectively improve CD and relieve the symptoms of CWI in anterior circulation.
