参附注射液改善慢性心力衰竭肺水肿理化指标的临床观察

摘　　要：目的:参附注射液改善慢性心力衰竭(CHF)患者脉搏指示心排血量监测(Pi CCO)参数、血浆脑钠肽(BNP)和氧合指数(Pa O2/Fi O2)的临床观察。方法:CHFⅣ级患者64例,随机分为观察组和对照组,各32例;对照组给予常规重症监护及综合治疗;观察组在前者基础上加用参附注射液治疗。所有患者均行Pi CCO监测,并在入院、48、96h时点记录参数,同期抽取动脉血检查血气和BNP值,比较两组临床疗效评分、胸腔内血容量指数(ITBVI)、血管外肺水指数(EVLWI)、心输血指数(CI)、BNP和Pa O2/Fi O2变化。结果:观察组临床疗效、中医证候疗效、Pi CCO参数、Pa O2/Fi O2等改善上较对照组有不同程度优势。结论:参附注射液通过Pi CCO参数及Pa O2/Fi O2更直接、连续地显示了其改善CHF肺水肿的临床疗效。Objective： To observe the effect of Shenfu Injection on the pulse indicate contour cardiac output （PiCCO）, BNP and oxygenation indexes of chronic cardiac failure （CHF）. Methods： Sixty-four patients with CHF grade IV were randomly divided into treatment group and control group, each group for 32 cases. The control group were received conventional intensive care and comprehensive treatment, while the treatment group were received Shenfu Injection combined with conventional intensive care and comprehensive treatment. All patients were received the PiCCO monitor on admission, 48hours and 96 hours after admission, meanwhile the values of blood gas analysis and BNP were detected. The indexes such as the score of clinical effects, ITBI index, EVLWI index, CI index and the contents of BNP in plasma and PaO2/FiO2 were observed. Results： The score of clinical effect, the score of TCM syndrome effect, PiCCO parameter and PaO2/FiO2 in the treatment group were improved, which were superior to those in the control group. Conclusion： Shenfu Injection can improve the clinical effect of CHF with pulmonary edema, the mechanism of which may be related to the improvment of the PiCCO parameter and PaO2/FiO2.
