摘 要:目的观察针刀疗法对腰椎间盘突出症根性神经痛大鼠血清白细胞介素-1(interleukin-1,IL-1)和白细胞介素-6(interleukin-6,IL-6)含量的影响,探讨针刀疗法治疗腰椎间盘突出症根性神经痛的机制。方法将40只雄性SD大鼠随机分为正常组、假手术组、模型组、针刀组,每组10只。采用非压迫性髓核突出模型,用热痛测试仪检测各组大鼠模型复制前后缩爪潜伏期;模型复制后第8天,采用双抗夹心ELISA法检测各组血清IL-1、IL-6含量。结果与正常组和假手术组比较,模型组缩爪潜伏期差值显著升高(P<0.01);与模型组比较,针刀组缩爪潜伏期差值显著降低(P<0.01)。与正常组比较,模型组血清IL-1、IL-6含量明显增加(P<0.01);与模型组比较,针刀组IL-1、IL-6含量明显减少(P<0.01)。针刀组大鼠神经根的髓鞘、轴突和雪旺细胞的病理变化较模型组明显减轻。结论针刀疗法减轻腰椎间盘突出症根性神经痛的机制与其降低血清炎性细胞因子IL-1和IL-6有关。Objective To observe the effect of acupotomy on serum interleukin-1 (IL-1) and interleukin-6 (IL-6) levels in rats with discogenic radiculagia and to explore the possible mechanism of acupotomy in treating this disease.Methods Forty male Sprague-Dawley rats were randomly divided into normal group,sham operation group,model group,and acupotomy group,with 10 rats in each group.A rat model of non-compressive nucleus pulposus protrusion was induced.A thermal pain tester was used to measure the hindpaw withdrawal latency (HWL) of each rat before and after the model was induced.On the 8th day after the model was induced,double-antibody sandwich enzyme-linked immunosorbent assay was used to determine serum IL-1 and IL-6 levels in each group.Results Compared with the normal group and sham-operation group,the model group had a significantly increased difference value of HWL (P<0.01) ; compared with the model group,the acupotomy group had a significantly reduced difference value of HWL (P<0.01).Compared with the normal group,the model group had significantly increased serum IL-1 and IL-6 levels (P<0.01); compared with the model group,the acupotomy group had significantly reduced serum IL-1 and IL-6 levels (P<0.01).Compared with the model group,the acupotomy group had ameliorated pathological changes in the myelin sheath,axon,and Schwann cells.Conclusion The mechanism by which acupotomy relieves radicular pain due to lumbar disc herniation may involve reducing serum IL-1 and IL-6 levels.
