摘 要:目的 应用血氧水平依赖功能磁共振成像(BOLD-fMRI)技术,分析正常成人痛觉相关皮层中枢的分布及量化特征.方法 选取10名正常成人志愿者,测定其痛阈并给予利手和非利手腕部2倍痛阈的电刺激,同时进行脑功能BOLD-fMRI扫描,应用SPM5进行后处理获得脑功能图像和数据,对脑激活区进行定位并测定其体积、强度.结果 刺激利手时激活对侧Brodmann Area(BA)3、1、2区7例,BA40区5例,BA5、7区3例,BA9、10、11区3例,BA6、8区2例,激活同侧BA5、7区2例,BA9、10、11区2例,BA6、8区3例;刺激非利手时激活对侧BA3、1、2区7例,BA40区6例,BA5、7区3例,BA9、10、11区2例,BA6、8区1例,激活同侧BA5、7区3例,BA9、10、11区1例,BA6、8区2例.3例在刺激两侧时BA3、1、2区和BA5、7区均没有激活,刺激利手时激活对侧BA40区2例、BA6区2例、前额叶皮质1例,刺激非利手时激活对侧BA40区2例、BA6区1例、前额叶皮质1例.结论 痛觉刺激可同时激活一般躯体感觉中枢、缘上回、辅助运动区、边缘系统等相关的皮层中枢,从而引发情绪、认知和内脏系统以及躲避反应等一系列相关活动.Objective To analyze the pain related cortical distribution and quantify features of normal adults by blood oxygen level dependent functional magnetic resonance imaging (BOLD-fMRI) technology.Methods A total of 10 normal adult volunteers were selected to measure their pain threshold,and then were given electrical stimulation of twice pain threshold of dominant and non-dominant hand.BOLD-fMRI was used to scan the brain function at the same time.The images were post-processed by SPM5 to get image and data of brain function for positioning and measuring the volume and intensity of brain activation area.Results When stimuluing dominant hand,the situation of activated Brodmann Area(BA) of contralateral cortex were as following:7 cases for BA 3,1 and 2,5 cases for BA 40,3 cases for BA 5 and 7,3 cases for BA 9,10,11,2 cases for BA 6,8.Meanwhile,The situation of ipsilateral cortex were activated were 2 cases for BA 5,7,2 cases for BA 9,10,1 1 area,3 case for BA 6,8.When stimulus were on non-dominant hand,the activated BA of contralateral cortex were as follow:7 cases for 3,1,2 area,6 cases for BA 40 area,3 cases for BA 5,7 area,2 cases for BA 9,10,11 area and 1 cases for BA 6,8 area.Furthermore,3 cases were BA 5,7 area,1 cases for BA 9,10,11 area,2 cases for BA 6,8 area in terms of ipsilateral cortex.To our surprise,the BA 3,1,2 area and BA 5,7 area of 3 cases were not activated when stimulated both sides.The contralateral BA 40 area (2 cases),BA6 zone (2 cases),the prefrontal cortex(1 case) were activated when the dominant hand was stimulated.While stimulation of the non-dominant hand,2 cases were activated contralateral BA 40 area,1 case for BA 6 area and 1 case for the prefrontal cortex.Conclusion The pain stimulation simultaneously activate general somatic sensory,supramarginal gyrus,supplementary motor area and the limbic system,and then led to a series of related activations such as emotional,cognitive,visceral systems and the escape response.
