维格列汀对新诊断2型糖尿病的疗效及对血糖波动的影响研究

摘　　要：目的探讨维格列汀治疗新诊断2型糖尿病的疗效及对血糖波动的影响。方法选择2012年3月—2013年8月在北京军区总医院住院的新诊断2型糖尿病患者68例,采用随机数字表法将患者分为两组:二甲双胍组35例,维格列汀组33例。所有患者均给予常规的饮食及运动治疗,在此基础上,二甲双胍组给予二甲双胍0.5 g,3次/d;维格列汀组给予维格列汀50 mg,2次/d,共治疗12周。比较两组治疗前后的空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)水平,治疗后的血糖标准差(SDBG)、日内平均血糖波动幅度(MAGE)、血糖波动最大幅度(LAGE)、平均餐后血糖波动幅度(MPPGE),并比较不良反应发生率。结果两组治疗前FPG、2 hPG、HbA1c各项指标间差异均无统计学意义(P>0.05);治疗后以上各项指标间差异亦均无统计学意义(P>0.05);治疗后,两组FPG、2 hPG、HbA1c与本组治疗前比较,差异均有统计学意义(P<0.05)。两组治疗后MAGE、LAGE、SDBG、MPPGE各项指标间差异均无统计学意义(P>0.05)。二甲双胍组不良反应发生率为25.7%(9/35),维格列汀组不良反应发生率为6.1%(2/33),两组比较差异有统计学意义(P=0.018)。结论对于新诊断2型糖尿病,维格列汀与二甲双胍控制血糖效果相当,但应用维格列汀患者不良反应更少。Objective This study aims to observe the efficacy of Vildagliptin and its influence on blood glucose fluctuation in treating newly diagnosed type 2 diabetes mellitus.Methods 68 cases of newly- diagnosed type 2 diabetics in General Hospital of Beijing Military Region from March 2012 to August 2013 were involved as subjects.They were randomly divided into two groups using random number table： Metformin group（ 35 cases） and Vildagliptin group（ 33 cases）.Conventional diet and exercise therapy was applied to every subject,on basis of this,Metformin 0.5 g tid and Vildagliptin 50 mg bid were given to Metformin group and Vildagliptin group for 12 weeks respectively.The changes in FPG,2 hPG,HbA1 c,as well as SDBG,MAGE,LAGE,MPPGE and adverse effects rate were observed between the two groups.Results There was no statistical significance in the differences of FPG,2 hPG,HbA1 cbetween two groups both before and after the treatment（ P 0.05）.Post-treatment FPG,2 hPG,and HbA1 cof the two groups were significantly different,compared with their pre- treatment levels of the same group（ P 0.05）.After the treatment,there was no statistical significance in differences of MAGE,LAGE,SDBG and MPPGE between the two groups（ P 0.05）.The adverse effects rate were 25.7%（ 9 /35） and 6.1%（ 2 /33） in Metformin group and Vildagliptin group respectively,and there was statistical significance in such difference between two groups（ P =0.018）.Conclusion Vildagliptin has the same effect as Metformin in controlling blood glucose,and produces less adverse effects in treating newly- diagnosed type 2 diabetes mellitus.
