房间隔缺损直径大小与预后的关系研究

摘　　要：目的：分析小儿房间隔缺损（ ASD）直径大小与预后的关系。方法随访91例ASD患儿的临床资料，根据ASD直径的大小分为三组，A组ASD直径0．5～0．7 cm，B组ASD直径＞0．7～1．0 cm，C组ASD直径＞1．0 cm。根据患儿年龄的大小分组，≤1岁患儿为D组，＞1岁患儿为E组。统计ASD直径及患儿年龄与自然愈合的关系。结果 A组共13例，其中自然愈合9例（69．23％）；B组33例，其中自然愈合9例（27．27％）；C组44例，其中自然愈合1例（2．27％）。 A组自然愈合率均高于B组和C组（χ^2＝6．89、27．22，均P＜0．05），B组高于C组（χ^2＝8．57，P＜0．05）。 D组有29例，其中自然愈合27例（93．10％），E组自然愈合17例（27．42％），两组差异有统计学意义（χ^2＝34．14，P＜0．05）。结论 ASD直径≤0．7 cm及年龄≤1岁的ASD自然愈合的可能性较大。 ASD直径＞1．0 cm及1岁以上的ASD自然愈合机会很小。Objective To analyze the relationship between diameter and prognosis in atrial septal defect （ ASD） children.Methods The clinical data in 91 cases with ASD were observed.According to ASD diameter,they were divided into three groups,0.5-0.7cm in group A,〉0.7-1.0cm in group B,〉1.0cm in group C.According to the age of children with size,≤1 years old children with group D,group E,〉1 years old children.Statistical ASD diameter and age of the patients and the relationship between natural healing.Results A group of ASD diameter in 0.5-0.7cm 13 cases,the number of natural healing rate in 9 cases（69.23%） .Group B of ASD diameter of 0.8 to＆amp;nbsp;1.0 cm in 33 cases,the number of natural healing rate in 9 cases （27.27%）.44 cases in group C ASD〉1.0cm in diameter,in which 1 cases （natural healing rate 2.27%）.Group A was higher than that in group B and C （χ^2 =6.89,27.22,all P〈0.05）.B group than in C group （χ^2 =8.57,P〈0.05）.Group D patients older than 1 years were 29 cases,in which the natural healing rate in 27 cases （93.10%） ,higher than that of E group of 1 year old chil-dren＆#39;s natural healing rate was 17 cases （27.42%）,two groups had significant difference （χ^2 =34.14,P〈0.05）. Conclusion ASD diameter≤0.7cm and age〈1 ASD natural healing probability.ASD〉1.0cm in diameter and 1 years of age or ASD natural healing a very small chance.
