心房颤动患者生活质量及影响因素分析

摘　　要：目的:了解并分析心房颤动患者生活质量及影响因素。方法:采用SF-36量表及AFEQT量表,对233例于我院门诊就诊的心房颤动患者进行生活质量调查,分析影响其生活质量的因素。结果:心房颤动患者生活质量总分及各维度均显著低于健康人群组。女性患者、症状负荷越重、近1年因心房颤动急诊就诊次数越多、医疗报销比例越低、正接受华法林治疗,是导致患者生活质量总体显著受损的主要因素。结论:心房颤动患者生活质量较健康人群显著受损。性别、症状负荷、近1年因心房颤动的急诊就诊次数、医保类型与正接受华法林治疗,是现阶段影响心房颤动患者生活质量的主要因素。应该采取并加强相应的措施及治疗以提高房颤患者生活质量。Objective：We sought to elucidate the status of the quality of life （QoL） in patients with atrial fibrillation （AF） and its influencing factors. Methods：A Cross-sectional survey was carried out among 233 consecutive AF patients and 100 consecutive healthy people by using the Short Form （SF）-36 health survey and Atrial Fibrillation Effect on QualiTy-of-life （AFEQT）. Multiple stepwise regression analysis was run for all val- id data on affecting factors of QoL by using the SPSS package. Results ： It was shown that patients with AF have significantly poorer QoL across all domains compared with healthy controls. The single factor analysis indicate that QoL was influenced mainly by such factors as age, gender, type of medical insurance, course of AF, smoking history, emergency department visit and warfarin treatment. Female, symptomatic patients, emergency department visit in the past year, lower proportion of medical reimbursement are factors strongly associated with lower QoL scores after multivariable adjustment. Conclusion：AF has immense impact on QoL in patients with AF. In order to improve QoL in AF patients, we should pay more attention to female AF patients, AF symptoms control, social security system and education on warfarin treatment.
