摘 要:目的探讨嗜酸性粒细胞趋化因子(Eotaxin)和基质金属蛋白酶9(MMP-9)在息肉组织中的表达及其与嗜酸性粒细胞(Eos)活化和发病的关系。方法选择2010年7月至2013年7月在荆州市中心医院住院接受鼻息肉手术治疗的患者50例作为观察组,并选取35例鼻外伤及慢性鼻炎等接受手术的患者作为对照组。分别于鼻内镜下摘取观察组患者鼻内息肉组织和对照组患者鼻下甲黏膜组织进行苏木精-伊红染色和免疫组织化学SP染色,观察Eotaxin和MMP-9的表达及Eos浸润情况。结果观察组患者中血清嗜酸性粒细胞阳离子蛋白(ECP)水平显著高于对照组[(17.32±4.62)μg/L vs(3.28±1.22)μg/L],差异有统计学意义(P<0.01)。观察组患者Eotaxin阳性细胞数显著高于对照组[(47.25±13.18)个vs(12.48±10.12)个],差异有统计学意义(P<0.01)。MMP-9在对照组中呈弱阳性表达,而在观察组患者中MMP-9均呈强阳性表达,两组平均灰度值比较差异有统计学意义(P<0.01)。结论息肉组织中Eotaxin和MMP-9可通过增加Eos浸润,引发组织的异常重塑,在鼻息肉存在和发展中起重要作用。Objective To investigate the expression of eosinophil chemotactic factor and matrix metallo- proteinase-9 (MMP-9) in polyps and its relationship with eosinophils extremely (Eos) activation and morbidity. Methods A total of 50 patients with nasal polyps admitted in Jingzhou Central Hospital from Jul. 2010 to Jul. 2013 were chosen as the observation group, another 35 cases of patients with nasal trauma, chronic rhinitis were chosen as the control group. Polyp tissues of the patients in observation group were obtained under endoscopy, and A nasal tissues of patients in control group were also obtained under endoscopy, all the sam- ples were processed by HE staining and immunohistochemical SP staining, and the expression of eosinophil chemotactic factor(eotaxin) and MMP-9 as well as the eosinophil infiltration were observed. Results The serum eosinophil cationic protein( EPC ) level of the observation group was significantly higher than the control group [ ( 17.32±4.62)μg/L vs (3.28±1.22)μg/L], the difference was statistically significant ( P 〈 0.01 ). Eotaxin-positive cell counts in the observation group was higher than the control group [(47.25±13.18 ) vs ( 12.48±10.12 )], the difference was statistically significant ( P 〈 0; 01 ). Expression of MMP-9 in normal damage nasal (non-polyp) tissues was weakly positive,whereas expression of MMP-9 in patients of the observation group, showed strong positive expression, difference of the average gray value of the two groups was statistically significant( P 〈 0.01 ). Conclusion Eosinophil chemotactic factor and matrix metal- loproteinase-9 can increase eosinophil infiltration, causing the abnormal tissue remodeling, which plays an important role in the existence and development of nasal polyps.
