消胀贴神阙穴贴敷治疗脓毒症胃肠功能障碍的理论阐释

摘　　要：脓毒症是严重感染引起的全身炎症反应综合征,进一步发展可致多脏器功能衰竭(MODS)。脓毒症胃肠功能障碍既是MODS的局部器官表现,又是肠道黏膜屏障功能进一步破坏的启动因子,造成肠道内细菌和毒素移位,促使MODS恶化。本文结合古今文献对脓毒症胃肠功能障碍中医发病机制进行分析,归纳出脾胃升降失常是导致脓毒症胃肠功能障碍的重要病机特点,"升清降浊"是治疗脾胃病变的基本法则。采用消胀贴神阙穴贴敷可达到升清降浊、促进胃肠气机复常的功效,在一定程度上阻断MODS的进一步发展。Sepsis was a systemic inflammatory response syndrome induced by severe infection,and it＇s further development could cause multiple organ dysfunction syndrome（MODS）.The gastrointestinal dysfunction in sepsis was not only the local performance,but also the initial factor that could cause the further damage of intestinal mucosal barrier.The displacement of intestinal bacteria and toxin was induced by this process,and made the MODS get worse.Referring the ancient and modern literature,this article analyzed the TCM pathogenesis of gastrointestinal dysfunction.And this article induced the important characteristics for pathogenesis of gastrointestinal dysfunction in sepsis,which is the abnormal ascending and descending of qi resulting from the failure of the spleen and stomach to function properly.So the basic law to treat the spleen and stomach dysfunction was ascending usables and descending unusables.So the Shenque acupoint application combined with Xiaozhang paste had the function of promoting gastrointestinal qi activity and blocking the further development of MODS in a certain extent.This method had a good prospect of clinical application.
