改良银质针疗法治疗椎动脉型颈椎病疗效观察

摘　　要：目的:观察由本院独创的改良银质针疗法对椎动脉型颈椎病的临床疗效及对经颅多普勒(TCD)的影响。方法:112例椎动脉型颈椎病患者随机分成治疗组60例和对照组52例。治疗组采用我院自行研发的改良银质针外接温针仪进行治疗,对照组采用普通银质针加艾灸治疗。治疗30天后比较两组患者的临床主要症状、体征及TCD的变化。结果:治疗组总有效率为90.00%,对照组总有效率为92.31%,组间对比无显著性差异,但愈显率(痊愈+显效)之间存在显著性差异,治疗组优于对照组。治疗后两组患者的基底动脉与双椎动脉的Vs、Vd、PI三项均得到明显改善,与治疗前相比均有显著性差异(P<0.01);其中,治疗组在治疗后基底动脉Vs、基底动脉PI、双椎动脉Vd、右椎动脉PI方面与对照组相比有显著性差异(P<0.05),明显高于对照组。但治疗后两组患者的基底动脉与双椎动脉的RI与治疗前相比未发现有显著性差异(P>0.05)。结论:改良银质针治疗本病可显著提高愈显率,提高椎-基底动脉的血流速度,增加血管弹性,降低血管阻力,且具有操作简单,不产生烟雾,不易烫伤病人等优势,值得临床推广应用。Objective：To observe the clinical efficacy of cervical spondylosis arteriopathy （CSA） and the influence on tran scranial doppler （TCD） by treating with the improved sliver needling treatment which is invented by our hospital. Methods：112 patients with CSA were randomly divided into treatment group and control group. There were 60 patients in treatment group and 52 in control group. The treatment group was treated by the improved sliver needling and the needle would be connected with the warm needling machine. The control group was treated by common acupuncture with moxibustion. After 30 days treatment, we would compare the changes in clinical efficacy, signs and TCD between the two groups. Results：The total effective rate of the treatment group was 90.00% while the control group＇s was 92.31%. But the cured and markedly effective rate had significant difference. And the cured and markedly effective rates of treatment group were better than those of the control group. After treating, the Vs, Vd and PI of basilar artery and vertebral artery all became better. And there was significant difference before and after treatment P 〈 0. 01 ）. The treatment group had significant difference from the control group in posterior basilar artery＇s Vs,basilar artery＇s PI, both vertebral arteries＇ Vd and right vertebral artery＇s PI. And they were apparently higher than those of the control group. Compared before and after treatment, there was no difference in basilar artery and both vertebral arteries＇ RI （P 〉 0. 05 ）. Conclusion：The improved sliver needling treatment can improve the cured and markedly effective rates in treating CSA. And it can increase the blood speed which is in vertebral - basilar artery and the blood vessel resilience and reduce tile resistance of blood vessel. This operation is simple. It won＇t produce smoke, and it won＇t burn the patients. Because of these advantages, it is worthy of spreading.
