摘　　要：目的探讨非AIDS患者肺隐球菌病感染的临床特征,比较有无基础疾病的肺隐球菌病患者临床与病理学特征,以提高早期诊断准确率。方法本研究回顾性分析2009年1月-2012年11月间收治入院的21例非AIDS肺隐球菌病患者的临床资料、影像学表现与病理学特征。并比较有基础疾病者和无基础疾病者的差异。结果其中有基础疾病者(13例)的年龄较高,其临床症状较无基础疾病者明显增多(P<0.05)。虽无统计学上差异,但乳胶凝集试验(LA试验)较葡聚糖试验(G试验)的阳性率更高,病理上两者均倾向肉芽肿型表现。结论肺隐球菌病的临床表现和影像学特点呈非特异性,有基础疾病者,高龄伴临床症状明显者,需尽早行病理学检查以早期诊断。Objective To analyze the clinical appearances among non-AIDS patients with pulmonary cryp-tococcosis,and to compare the clinical and pathological characteristics between those cases with or without underlying diseases.Methods 21 non-AIDS patients with pulmonary cryptococcosis were retrospectively analyzed by reviewing their clinical documents,imaging features and pathological characteristics who were hospitalized from January,2009 to November,201 2.The differences of above three characteristics were compared between patients with or without un-derlying diseases.Results Compared to those without underlying diseases,the patients with underlying diseases had higher average age and more obvious clinical symptoms （P〈0.05 ）.The serum latex agglutination （LA）test was more sensitive than serum fungal （P〉0.05 ）.Tissue pathology in both was prone to granuloma.Conclusion The clinical appearance and radiology of pulmonary cryptococcosis are nonspecific.People with underlying diseases,ad-vanced age and/or obvious symptoms should undertake pathological examination for early diagnosis.
