摘 要:目的探讨COPD患者营养状况与病情分级及机械通气风险间的关系。方法测定159例COPD患者营养指标,记录肺功能FEV1/FVC、FEV1pred%、近一年内急性发作次数、m MRC分级及机械通气情况,并进行分组。结果 COPD患者BMI、白蛋白和淋巴细胞计数的营养不良发生率分别为51.9%、42.8%和48.4%。营养指标中BMI、白蛋白、前蛋白和淋巴细胞计数与FEV1/FVC、FEV1pred%均有相关性,且与病情严重程度相关(P<0.05),球蛋白与FEV1/FVC及FEV1pred%无相关性。急性加重高风险组营养较低风险组差(P<0.05)。机械通气组营养差于非机械通气组(P<0.05)。结论 COPD患者营养不良发生率高,且营养状态与肺功能进展、急性加重及机械通气风险相关。Objective To study the correlation of nutritional indicators to the risk of mechanical ventilation and disease classification in patients with chronic obstructive pulmonary disease (COPD).Methods Their nutri-tional parameters,acute episodes,mMRC classification,mechanical ventilation,FEV1 /FVC and FEV1 pred% were observed and recorded.Results The malnutrition rates of BMI,albumin and lymphocyte count in COPD patients were 52.2%,42.8% and 48.4% respectively.BMI,albumin,former protein and lymphocyte count were correlate with FEV1 /FVC and FEV1 pred%,as well as associated with disease severity (P〈0.05 ).There was no correlation between globulin and the levels of FEV1 /FVC and FEV1 pred%.The nutrition status was worse in the high-risk group than in the low-risk group (P〈0.05 ),and it was also worse in the mechanical ventilation group than in the non-me-chanical ventilation group (P〈0.05 ).Conclusion The incidence of malnutrition in COPD patients is high,which shows an effect on their prognosis,lung function and mechanical ventilation.
