儿童川崎病低蛋白血症与炎症反应及肝功能的关系研究

摘　　要：目的:探讨川崎病患儿低蛋白血症与炎症反应及肝功能的关系,为临床诊疗提供参考。方法:采用回顾性研究方法,选取2010年1月1日至2013年12月31日在我院住院的川崎病患儿196例,入院时行血常规、肝功能、C-反应蛋白(CRP)、血沉(ESR)、尿常规等检测,根据血清白蛋白(ALb)浓度分为低蛋白血症组(ALb<35 g/L)136例和ALb正常组(ALb≥35 g/L)60例,比较两组白细胞(WBC)、血红蛋白(HB)、CRP、ESR、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆汁酸等实验室检查结果。结果:低蛋白血症组的ESR、CRP高于ALb正常组,HB低于ALb正常组,差异均有统计学意义(P均<0.01);两组ALT、AST、总胆汁酸比较差异无统计学意义(P>0.05)。结论:儿童川崎病低蛋白血症的发生与炎症反应的剧烈程度有关,而与肝功能损害无明显关系。Objective： To investigate the association of hypoproteinemia in Kawasaki disease with inflammation response and liver function. Methods： Clinical datum of 196 cases of Kawasaki disease in Children＇s Hospital of Hunan Province from January 1,2010 to December 31, 2013 were analyzed retrospectively. All children were divided into hypoproteinemia group （ALb 〈35 g/L） and normal group （Alb ≥ 35 g/L） , according to serum albumin concentration. Laboratory test results were compared including leukocyte （WBC） , hemoglobin （HB）, CRP, ESR, alanine aminotransferase （ALT）, aspartate aminotransferase （AST）, total bile acid. Results： ESR, CRP and hemoglobin had significant difference between two groups （P〈0.01）. ALT, AST, total bile acid had no significant difference between two groups （ P〉0. 05 ）. Conclusion： Hypoalbuminemia of Kawasaki disease children associated with the level of inflammation response, which is unrelated to liver dysfunction.
