摘  要:目的:探讨孕中晚期母体体成分及体成分改变与新生儿出生体重的关系。方法:回顾性分析建卡、足月分娩的单胎健康孕妇65例在孕中期23-24周和孕晚期32-33周的人体成分与新生儿出生体重之间是否有相关性,新生儿出生体重按照分娩时称重数据记录。结果:65例孕前体重过轻9例,孕前体重正常54例,孕前超重2例;孕中晚期体成分分析中,母体的BMI、体脂肪与新生儿出生体重间差异无统计学意义(P〉0.05),而孕中期和晚期母体体重、BMR、去脂体重、骨骼肌肉量、全身肌肉量、蛋白质、矿物质、身体水分含量(细胞内液+细胞外液)、臀围均与新生儿出生体重有相关性(P〈0.05);而孕中期至晚期增长的体成分中,各成分均与母体体重的增加有关,但其中除体重、BMI、臀围的增加与新生儿出生体重相关外,其他均无相关性。结论:孕期体重的增加与母体各种体成分的改变有关,并且能影响胎儿的生长发育及出生体重,其中去脂体重是新生儿出生体重的一个重要预测因素。Objective: To explore maternal body composition during the second and the third trimesters of pregnancy and the rela- tionship between change of maternal body composition and neonatal birth weight. Methods: The correlation between maternal body composi- tion of 65 healthy pregnant women (single pregnancy, full -term delivery) during 23 -24 and 32 -33 gestational weeks and neonatal birth weight was analyzed retrospectively; neonatal birth weight was recorded. Results: Among 65 pregnant women, 9 women were underweight before pregnancy, weight before pregnancy of 54 women was normal, and 2 women were overweight; there was no statistically significant difference in maternal body mass index ( BMI), body fat and neonatal birth weight ( P 〉 0. 05 ) ; maternal body weight, BMI, lean body mass, skeletal muscle mass, muscle mass, protein, mineral substance, total body water ( including intraeellular fluid and extraeellular fluid) and hip circumference during the second and the third trimesters of pregnancy were correlated with neonatal birth weight ( P 〈 0. 05) ; mater- hal body composition during the second and the third trimesters of pregnancy was correlated with maternal body weight gain, but except the increases of body weight, BMI and hip circumference, there was no correlation between the other indexes and neonatal birth weight. Conclu- sion: Body weight gain during pregnancy is correlated with changes of maternal body composition, which can affect growth and development of fetuses and neonatal birth weight, lean body mass is an important predictive factor for neonatal birth weight.
