异位妊娠患者早期诊断中对血浆血管内皮生长因子测定的临床意义

摘　　要：目的：分析异位妊娠（EP）患者早期诊断中对血浆血管内皮生长因子（VEGF）测定的临床意义,为临床诊断提供参考。方法：选取2012年1月-2013年12月期间该院接受治疗并确诊的78例EP患者作为观察组,选择同期在该院门诊的78例正常宫内妊娠妇女作为对照组。血浆VEGF应用酶联免疫吸附试验（ELISA）进行测定;采用放射免疫法检测血浆孕酮（P）含量。结果：观察组测定P值（6.84±12.34）ng/ml显著低于对照组（27.35±12.06）ng/ml,且差异有统计学意义（P〈0.05）;观察组测定VEGF值（95.38±67.49）pg/ml显著高于对照组（17.37±11.57）pg/ml,且差异有统计学意义（P〈0.05）;以血浆P值62 nmol/L为截断点,诊断EP的灵敏度为96.25%,特异度为73.16%;以血浆VEGF值238 pg/ml为截断点,诊断EP的灵敏度为83.49%,特异度为91.58%。结论：EP早期诊断中可将血浆VEGF含量作为新的一种辅助生化指标,该项指标是了解患者病情及判断预后的指标之一,对于临床早期诊断EP具有重要研究价值,值得进一步应用。Objective： To analyze the clinical significance of plasma vascular endothelial growth factor （VEGF） detection for early diagnosis of ectopic pregnancy, provide reference for clinical diagnosis of this disease. Methods ： A total of 78 patients with ectopic pregnan- cy treated in this hospital from January 2012 to December 2013 were selected as observation group, 78 normal pregnant women during the same period were selected from outpatient department of this hospital as control group; ELISA was used to detect plasma VEGF level; radio- immunoassay was used to detect plasma progesterone level. Results： Plasma progesterone level in observation group was （6. 84 ~ 12. 34） ng/ml, which was statistically significantly lower than that in control group [ （27.35 ± 12. 06） ng/ml ] （P 〈 0.05 ） ; plasma VEGF level in observation group was （95.38 ±67.49） pg/ml, which was statistically significantly higher than that in control group [ （17.37 ± 11.57 ） pg/ml ] （ P 〈 0.05 ） ; taking plasma progesterone level 62 nmol/L as cut - off point, the sensitivity and specificity of diagnosis of ec- topic pregnancy were 96. 25% and 73.16%, respectively; taking plasma VEGF level 238 pg/ml as cut - off point, the sensitivity and specificity of diagnosis of ectopic pregnancy were 83.49% and 91.58%, respectively. Conclusion： During early diagnosis of ectopic pregnancy, plasma VEGF level can be used as a new adjunct biochemical marker, which is one of the indicators to understand the patients＇condition and predicting prognosis, the research value for early diagnosis of ectopic pregnancy is significant and the method is worthy further application.
