摘 要:目的:探讨异丙酚注射液和微量肝素对新生大鼠高氧肺损伤的影响,为高氧肺损伤的临床治疗提供理论依据。方法:选取Wistar乳鼠120只,随机分为空气对照组、高氧模型组、异丙酚治疗组和微量肝素治疗组,每组各30只。实验开始第3、7及14天各处死10只动物,收集支气管肺泡灌洗液(BALF),采用硫代巴比妥酸比色法测定丙二醛(MDA)水平,取肺组织测定肺湿/干质量比(W/D),观察肺水肿情况,采用HE染色观察肺组织的病理变化,采用免疫组化染色观察肺组织中TNF-α的表达和Clara细胞的数量。结果:异丙酚治疗组BALF中MDA水平、肺W/D、肺组织中TNF-α的表达强度明显低于高氧模型组,Clara细胞数明显高于高氧模型组,差异均有统计学意义(P均<0.01);但各药物之间比较,差异无统计学意义(P>0.05)。结论:异丙酚注射液及微量肝素可以减轻新生大鼠高氧肺损伤程度,对高氧环境下的肺组织具有一定的保护作用。Objective: To explore the effects of propofol injection and trace heparin on hyperoxic lung injury in neonatal rats, pro- vide a scientific basis for clinical treatment of hyperoxic lung injury. Methods: A total of 120 neonatal Wistar rats were selected and ran- domly divided into air control group, model group, propofol group and trace heparin group, 30 rats in each group. Ten rats in each group were put to death on the third day, the seventh day and the fourteenth day after beginning of the experiment to collect bronchoalveolar lavage fluid (BALF) specimens; thiobarbituric acid chromatometry was used to detect the level of malondialdehyde (MDA) ; lung specimens were ob- tained to detect lung wet -to -dry weight ratio (W/D) and observe pulmonary edema; HE staining was used to observe pathological changes of lung tissue; immunohistochemical staining was used to observe the expression of tumor necrosis factor - α (TNF - α) and the number of Clara cells. Results : The expression level of MDA in BALF, lung W! D and the expression level of TNF - α in lung tissue in propofol group were lower than those in model group, while the number of Clara ceils in propofol group was higher than that in model group, there were statistically significant differences (P 〈0. 01 ) ; but there was no statistical- ly significant difference between propofol group and trace heparin group (P 〉 0. 05) . Conclusion : Propofol injection and trace heparin can relieve the degree of hyperoxic lung injury in neonatal rats, which can protect lung tissue under hyperoxic environment to a certain extent.
