经口气管插管患者口腔护理频次的研究

摘　　要：目的探讨经口气管插管行机械通气患者口腔护理的合理频次。方法将经口气管插管行机械通气患者60例随机分为A、B两组,每组30例,应用牙刷刷牙后,A组患者用生理盐水进行口腔护理,B组患者用0.12%醋酸氯己定进行口腔护理,分别采集两组患者口腔护理前、口腔护理后30min、2h、4h、6h、8h口咽部标本进行细菌计数。结果 B组患者口咽部菌落数明显较A组患者少(P=0.000);但机械通气2h后,细菌总数和口腔护理前比较差异无统计学意义(P=0.262)。A组患者机械通气4h、6h、8h后细菌较操作前差异有统计学意义(P>0.05)。结论用0.12%的醋酸氯己定口腔护理,能够显著降低经口气管插管患者口咽部细菌黏附的数量,A组患者口腔护理后细菌生长速度快于B组;口腔护理后2h,患者口咽部细菌总数就达到操作前水平,间隔越短时间进行口腔护理,越有利于控制口咽部的细菌数。Objective To study the oral care frequency for patients with orotracheal intubation. Method 60 pa- tients with orotracheal intubation undergoing mechanical ventilation were randomly divided into two groups （n=30）, Group A as control group gargled Sodium Chloride, group B gargled with 0.12% Chlorhexidine gluconate, which all adopted brushing the teeth. The oropharyngeal specimens was collected 30 rain, 2 hours, 4 hours, 6 hours and 8 hours after and before oral care, and then counted bacteria colonies. Result Colony count for patients in group B was significant difference compared with patients in group A（P=0. 000）. Colony count of 2 hours after care had not significant difference compared with that before oral care（P=0. 262）. Colony count in 4 hours, 6 hours, 8 hours after oral care in group A was significant difference compared with that in group B（P〉0.05）. Conclusion 0. 12% Chlorhexidine gluconate are effective to decrease oropharyngeal bacterial colonization for patients with orotracheal in- tubation, bacterial growth rate of Sodium Chloride group is greater than that Chlorhexidine gluconate group. Colony count of 2 hours after oral care is the same as before, It＇s recommended that the shorter frequency of oral care can help to control the Colony count of the oropharyngeal bacteria.
