快速康复外科理念在泌尿外科后腹腔镜手术患者围术期护理中的应用

摘　　要：目的探讨快速康复外科理念在泌尿外科后腹腔镜手术患者围术期护理中的应用效果。方法将本院泌尿外科收治的91例行后腹腔镜手术患者随机分为观察组46例,对照组45例,观察组患者实施快速康复外科理念(fast track surgery,FTS)指导的围术期护理措施,包括术前饮食、肠道管理、术后保温、活动与饮食管理、输液管理及导尿管处理等内容,对照组采用传统围术期护理措施,比较二组患者术后首次排便、排尿时间,术后住院费用、住院时间及并发症情况。结果 1观察组术后首次排便、排尿时间、住院时间均低于对照组,且差异均具有统计学意义(t=5.515,P<0.000;t=4.608,P<0.000;t=2.923,P=0.004)。2二组患者术后住院费用差异无统计学意义(t=0.350,P=0.727);3观察组与对照组术后出现并发症患者构成比分别为32.61%(15/46)、53.33%(24/45),差异有统计学意义(χ2=4.090,P=0.043)。结论泌尿外科后腹腔镜手术患者围术期采用FTS理念指导下的护理措施,可明显促进患者康复,且安全有效。Objective To explore and discuss efficacy of fast track surgery in perioperative nursing on patients with retrooperitoneoscopic surgery.Methods A total of 91 admitted cases with retrooperitoneoscopic surgery in department of urology were randomly divided into observation group（46 cases）and control group（45 cases）,the patients in observation group were applied with fast track surgery（FTS）guidance of peri operation nursing,which including preoperative diet,bowel management,postoperative insulation,activity ＆ diet management,transfusion ＆ preicatheter management,while control group was given traditional perioperative nursing measures,and postoperative defecation time,urination time,postoperative hospital expenses,hospital stays and complications were noted and compared between two groups.ResultsPatients in observation group whose first defecation time,urination time,hospital stays were all lower than control group,and the differences were statistically significant（t=5.515,P〈0.000;t=4.608,P〈0.000;t=2.923,P=0.004）.Postoperative hospitalization expenses of patients had no significant difference between two groups（t=0.350,P=0.727）.Complication incidences of observation group and control group were 32.61%（15/46）and 53.33%（24/45）respectively,the difference was statistically significant（χ2=4.090,P=0.043）.Conclusion Nursing measures under the guidance of FTS given to patients with retrooperitoneoscopic surgery can obviously promote patients’ early recovery,and it is also safe and effective.
