摘 要:目的通过建立放射性肝纤维化大鼠模型,探讨血清标志物透明质酸、Ⅳ型胶原、层粘连蛋白与肝纤维程度的相关性,寻找放射性肝纤维化较佳血清标志物。方法雄性SD大鼠40只,随机分为模型组(30只)和对照组(10只)。除对照组外,模型组大鼠右半肝均接受单次6MV-X线25 Gy照射,建立放射性肝纤维化模型。在模型组接受照射第2月、4月、6月后,随机抽取模型组大鼠10只、对照组3~4只,放射免疫法检测血清透明质酸、Ⅳ型胶原、层粘连蛋白值,同时进行大鼠肝组织HE染色,观察肝组织病理变化及大鼠肝纤维化程度,将后者与血清透明质酸、Ⅳ型胶原、层粘连蛋白值分别进行相关性分析。结果在照射第2月、4月、6月后,模型组:血清透明质酸分别为(208.23±41.23)μg/L、(269.00±59.21)μg/L、(320.82±67.53)μg/L,Ⅳ型胶原分别为(17.83±4.03)μg/L、(22.31±4.61)μg/L、(27.19±5.43)μg/L、层粘连蛋白值分别为(101.88±38.23)μg/L、(142.31±48.21)μg/L、(189.29±55.13)μg/L,均逐渐升高,且均明显高于对照组的(145.11±45.78)μg/L、(13.45±3.78)μg/L、(58.89±20.78)μg/L,F值分别为19.46、17.14、17.13,P〈0.05。通过相关性分析表明,大鼠血清透明质酸、Ⅳ型胶原、层粘连蛋白值与肝纤维化程度正相关(r=0.787、0.715、0.682,P〈0.05)。结论透明质酸、Ⅳ型胶原、层粘连蛋白可作为放射性肝纤维化严重程度的监测指标,其中以透明质酸的诊断价值最高。Objective To establish the rats model of radiation-induced hepatic fibrosis, explore the correlation between ra- diation-induced hepatic fibrosis and the serum levels of hyaluronic acid, typeⅣ collagen and laminin in rats, and look for better serum markers for radiation-induced hepatic fibrosis. Methods Forty healthy male Sprague-Dawley rats were ran- domly divided into model group(30 rats) and control group( 10 rats). The right-haft of rat liver in the model group was ir- radiated by 6-MV X-ray with a single dose of 25 Gy to establish radiation-induced hepatic fibrosis model. After irradiation at different time points(2 months ,4 months ,6 months), ten rats in model group, three or four rats in control group were sacrificed at each time point. The liver tissues from the irradiated liver, and blood sample were collected. The expression of hyaluronic acid, type Ⅳ collagen and laminin were tested by radioimmunoassay (RIA). Pathomorphological change in the liver was observed with hematoxylin-eosin staining by optical microscope. Results In model group on 2 months ,4 months and 6 months, the expression of serum hyaluronic acid was ( 208.23 ± 41.23,269.00 ±59.21,320.82 ± 67.53 ) μg/L, type Ⅳ collagen was( 17.83 ± 4.03,22.31 ± 4.61,27.19 ± 5.43 ) μg/L and laminin was ( 101.88 ± 38.23 )μg/L, 142.31 ± 48.21 μg/L, 189.29 ± 55.13 μg/L), which was significantly higher than the control group (145.11 ± 45.78 μg/L, 13.45 ± 3.78 μg/L,58.89 ± 20.78μg/L, F = 19.46,17.14,17.13, P 〈 0.05 ). There were positive correlation between the serum of hyaluronic acid, type Ⅳ collagen and laminin value and severity of hepatic fibrosis ( r = 0. 787, 0.715,0. 682, P 〈 0.05 ). Conclusion The hyaluronic acid, type Ⅳ collagen and laminin play important roles in the development of radiation-induced hepatic fibrosis. The serum hyaluronic acid was the parameter that alone presented the best diagnostic accuracy in the assessment of radiation-induced hepatic fibrosis.
