摘  要:目的探讨C臂X光机定位方法的改进在脊柱手术中的应用效果,为脊柱外科手术的治疗提供更好的方法,减少因定位不准而造成手术失败的发生率。方法选取台州市立医院2011年12月—2013年12月手术治疗的68例脊柱病变且需要手术的患者进行研究,按照临床试验随机的原则将患者分为2组,每组34例,对照组采用常规的X线检查方法进行定位,观察组采用C臂X光机改进的方法定位目标脊柱上缘并标记,分别测量两标记点距离该中心的距离、定点成功率、记录两种定位方法所花费的时间以及对工作人员的损伤程度等,观察2组方法的效果及相关指标,并对其结果进行统计学的分析。结果常规定位法定位所需时间平均(72.1±7.9)min,改良方法定位法为(18.2±3.8)min,两者相比,差异有统计学意义(t=21.078,P<0.05);常规定位法在定位准度方面,常规定位法所标记点距参考点的距离为(47.2±4.8)mm,改良定位法为(31.3±3.7)mm,两者相比,差异有统计学意义(t=10.673,P<0.05);采用C臂X光机定位方法的改进有助于减轻巡回护士身体的疲劳、减少工作人员的硬损伤,观察组患者放射反应发生率显著低于对照组,差异有统计学意义(χ2=7.917,P<0.05)。结论C臂X光机方法的改进较传统定位方法具有定位准确且迅速的优势,能够提高脊柱病变的定位的准确度,避免不必要的损伤,值得临床推荐。Objective To study the ways to improve the C-arm X-ray machine positioning method in spine surgery, pro- vide a better way for the treatment of spinal surgery, and reduce the incidence of failure of the surgery. Methods 68 pa- tients with spine surgery from December 2011 to December 2013 in our hospital were to be studied. In accordance with the principles of randomized clinical trials, the patients were divided into two groups with 34 cases in each group. The control group received the conventional X-ray examination method for positioning, while the observation group received the im- proved positioning methods to target the spine edge and mark the two markers. The distance from the center point to the markers was measured. The success rate of positioning, time for the positioning, the extent of damage to staff, the efficacy of positioning and related indicators were compared between the two groups. The results were statistically analyzed. Re- sults The time for the positioning for the conventional method was average time (72.1 ± 7.9) min, and for the improved methods was ( 18.2 ±3.8 ) min, the difference was statistically significant ( t = 21. 078, P 〈 0.05 ) ; The distance from the center point to the markers for the conventional method was(47.2 ±4.8) mm,and for the improved method was(31.3± 3.7 ) mm, the difference was statistically significant( t = 10.673, P 〈 0.05 ) ;the damage to the hard work of staff, the inci- dence of radiation reaction in patients was lower in the improved method group as compared with the conventional method group, the difference was statistically significance (χ2 = 7. 917, P 〈 0.05 ). Conclusion As compared with the conven- tional method, the improved C-ann X-ray machine positioning method shows the advantages of precise and quick positio- ning, this will improve the accuracy of the positioning of lesions, and avoid the unnecessary damage.
