电针不同波形联合康复训练治疗脑卒中迟缓期上肢功能障碍的疗效观察

摘　　要：目的观察、对比连续波、疏密波、断续波不同波形电针联合康复训练治疗脑卒中迟缓期上肢功能障碍的疗效。方法将69例脑卒中迟缓期患者按1∶1∶1的比例,随机分为断续波组23例,疏密波组23例,连续波组23例。3组患者均予相应的常规内科治疗,在生命体征平稳,不造成病情恶化的前提下,给予电针和康复训练。电针患侧上肢取肩、肩、臂、曲池、手三里、外关、合谷。连续波组采用连续波,频率15 Hz;疏密波组采用疏密波,频率2 Hz/15 Hz;断续波组采用断续波,频率15 Hz;强度以患者能够耐受为度,每次通电治疗30 min,1次/d,6次/周。3周后评定疗效。分别于治疗前后采用Fugl-Meyer评定法、改良Barthel指数评分评定上肢运动功能、日常生活活动能力,并比较3组疗效。结果断续波组疗效明显优于疏密波组、连续波组(P<0.01),疏密波组优于连续波组(P<0.05)。治疗后3组Fugl-Meyer评分、改良Barthel指数评分均显著改善(P<0.01),且断续波组优于疏密波组(P<0.05),优于连续波组(P<0.01)。结论电针断续波联合康复训练治疗脑卒中迟缓期上肢功能障碍效果优于电针疏密波、电针连续波,可以有助于早期诱发迟缓期上肢肌张力的出现,缩短迟缓期,显著改善上肢运动功能。Objective To investigate and compare the curative effect of different waveforms of acusector such as continuous wave,dilatational wave,and discontinuous wave combined with rehabilitation training to treat upper limb dysfunction in lag phase of stroke. Methods 69 patients of limb dysfunction in lag phase of stroke were randomly divided into discontinuous wave group,dilatational wave group,and continuous wave group with 23 cases in each group according to the ratio of 1 ：1：1.Three groups were given corresponding conventional medical treatment,and acusector combined with rehabilitation training was executed with steady vital signs and without exacerbation.The electroacupuncture points of the affected side were Jianyu,Jianliao,Binao,Quchi,Shousanli,Waiguan,and Hegu. The continuous wave group were treated with continuous wave of 15 Hz frequency,the dilatational wave group were treated with dilatational wave of 2 Hz/15 Hz frequency,and the discontinuous wave group were treated with discontinuous wave of 15 Hz frequency which was endurable by the patients. Each time the electroacupuncture lasted for for 30 min,once a day,six times a week and totally for 3 weeks.The Fugl-Meyer rating method and modified Barthel index score were adopted for assessment of the upper limb motor function and activities of daily living before and after treatment respectively,and the curative effects of the three groups were compared. Results The curative effects in the discontinuous wave groupwere clearly superior to those in the dilatational wave group and the continuous wave group（P0.01）,and the dilatational wave group was superior to the continuous wave group（ P〈 0.05）.After the treatment,Fugl-Meyer score and modified Barthel index score in the three groups were obviously improved（ P〈 0.01）.The discontinuous wave group was superior tp the dilatational wave group（ P〈 0.05）,and the dilatational wave group was superior to the continuous wave group（P0.01）.Conclusion The effects of discontinuous waveforms of acusect
