76例晚期肾癌患者采用索拉非尼治疗过程中出现的不良反应及护理措施

摘　　要：目的 观察应用索拉非尼治疗肾癌晚期的临床效果和护理服务满意程度,并针对治疗过程中发生的不良反应和相应的护理措施进行分析和探讨,以期给予临床一定的指导。方法 选择2010年6月~2013年6月就诊于哈尔滨医科大学附属肿瘤医院的肾癌晚期患者76例作为观察对象,针对应用索拉非尼治疗过程中出现的不良反应进行回顾性分析,并对其进行积极护理措施干预,对比两组疗效、药物不良反应及护理服务质量。结果 在治疗过程中死亡4例,其中2例因急性心肌梗死死亡,2例在用药开始16周内死亡,临床受益率为52.78%;手足综合征伴皮疹、高血压、腹泻及乏力发生率较高,经适当护理干预措施不良反应症状均有好转,未发生严重后果;72例患者中对护理服务满意57例（79.2%）,比较满意16例（22.2%）,不满意3例（4.2%）,护理服务满意度较高。结论 晚期肾癌患者应用索拉非尼疗效显著,但均发生不同程度的不良反应,症状均较轻微,通过积极的护理干预均有好转,患者均可耐受,患者对护理服务满意度较高。Objective To observe the clinical effect and nursing satisfaction of application of Sorafenib for treatment of advanced renal cell carcinoma, and analyze and discuss the adverse reactions and corresponding nursing measures during the treatment course to give some guidance for clinical work. Methods 76 patients with advanced renal cell carcinoma were chosen as research objects. Adverse reactions occurred during treatment of Sorafenib were retrospectively analyzed, and positive nursing measures were taken. Clinical effect, adverse reaction and quality of nursing service of two groups were compared. Results During treatment process, there were 4 patients died, among which 2 patients died of acute myocardial infarction and 2 patients died within 16 weeks of medication, the clinical benefit rate was 52.78%;occurrence rate of hand-foot syndrome complicated with rash, hypertension, diarrhea and weakness was high, by proper nursing intervention measures, all adverse reactions were relieved and no serious consequences happened. Among 72 patients, 57 patients（79.2%） were satisfied with nursing service, 16 patients（22.2%） were fairly satisfied and 3 patients（4.2%） were not satisfied, the nursing satisfaction degree was high. Conclusion Sorafenib for treatment of patients with advanced renal cell carcinoma has an obvious clinical effect, but all patients have different extents of adverse reactions which are all moderate symptoms, and by positive nursing interventions they can all be relieved and tolerable to patients. Patients are highly satisfied with nursing services.
