胰管空肠黏膜吻合四点法在胰肠吻合术中的应用

摘　　要：目的探讨胰十二指肠切除术胰肠吻合技术改进预防胰瘘的方法。方法报告并分析2010年6月至2013年3月,开展的23例胰管空肠黏膜吻合四点法预防胰瘘的临床经验和手术方法。结果 23例患者术后无胰瘘,发生胃瘫2例,炎性肠梗阻1例,急性胃黏膜病变1例,腹腔感染2例(腹腔引流液淀粉酶正常,排除胰肠吻合口瘘)。均保守治愈,痊愈出院。结论胰管空肠黏膜吻合四点法操作简便,吻合牢靠,胰液引流通畅,有效预防了胰瘘的发生,具有临床推广应用的价值。Objective To compare the methods of pancreaticojejunostomy in pancreaticoduodenectomy in the prevention of pancreatic fistula. Methods Analyzed clinical data and surgical methods of 23 four - point method pancreaticoduode- nectomy patients between June 2010 to March 2013. Results Totally 23 patients with no postoperative pancreatic fistu- la, and 2 cases of gastroparesis, 1 case of inflammatory bowel obstruction, acute gastric mucosal lesions in 1 case ,2 cases of abdominal infection（ peritoneal drainage fluid amylase normal, excluding pancreatic anastomotic fistula） , and all patients were conservative cured. Conclusion Four - point method pancreaticojejunostomy is easy to operate and secure in anastomosis, which is an effective Way of prevent pancreatic fistula, and have the value of clinical application.
