宫颈液基细胞检查联合阴道镜检查在宫颈病变中的应用价值

摘　　要：目的探讨宫颈液基细胞检查联合阴道镜检查在宫颈病变中的应用价值。方法收集无锡市妇幼保健院妇产科2013年3月至2014年3月门诊就诊疑似宫颈病变的妇科患者150例,按照随机数字表法分为联合组和阴道镜组,每组75例,阴道镜组实行单纯的阴道镜检查,联合组首先经过液基细胞学染色检查,再将筛出的阳性结果行阴道镜检查,将检查的结果和病理组织检查金标准比较,评价液基细胞检查联合阴道镜检查的临床的价值。结果联合组和阴道镜检查组总的阳性检出率为34.0%,联合组的阳性率为54.67%,明显的高于阴道镜组的阳性率13.33%,两组阳性率之间的差异具有显著的统计学意义(P<0.05);经过计算联合组诊断宫颈病变的符合率为53.33%(40/75),对于CIN I的符合率为54.05%(20/37),对于CIN II诊断的符合率为41.17%(7/17)。结论宫颈液基细胞联合阴道镜检查与病理组织检查具有一定的符合率,并且检查方法简单易行,值得临床上采纳使用。Objective To study the application value of cervical liquid based cell combined colposcope examination in cervical lesions. Methods Collected the hospital of department of gynaeeology in March 2013 to March 2014 outpatient or treatment after pathologic diagnosis of gynecological patients 150 cases, according to random number table method into the joint and vaginal lens group,75 cases in each group, colposcope a simple set of colposcopy examination, the joint group of first pass,basal cell staining to sieve out the positive results of colposcopy examination, will examine and his- topathologic examination gold standard comparison, the result of the evaluation of liquid based cell examination combined colposcope examination of clinical value. Results Joint group and colposcope team overall positive detection rate was 34. 0% , the joint group of positive rate was 54. 67% , significantly higher than vaginal lens group and the positive rate of 13.33% ,the difference between the two group of positive rate has significant statistical significance （P 〈 0. 05 ） ;The calculated joint group diagnosis coincidence rate was 53.33% （ 40/75 ） of cervical lesions. Calculation for CINIeoinci- dence rate was 54. 05% （20/37）, for CIN II diagnostic coincidence rate was 41.17% （7/17）. Conclusion Cervical liquid based cell combined colposcope examination and histopathologic examination has a certain coincidence rate, and check method is simple and worthy of clinical adoption.
