吗啡经硬膜外PCA输注对带状疱疹后神经痛NRS的影响

摘　　要：目的:探讨吗啡经硬膜外患者自控镇痛(patient-controlled analgesia,PCA)输注治疗带状疱疹后神经痛,对数字评分量表(numerical rating scales,NRS)的影响。方法:38例腹部带状疱疹后神经痛患者随机分为两组,两组患者均经胸椎12腰1椎间隙置入硬膜外导管,连接装有复合镇痛药液的PCA泵。A组(16例)镇痛液中不含吗啡;B组(22例)镇痛液中含有吗啡。采用数字评分量表(NRS)进行疼痛评估。结果:治疗第3、5、7天及出院后1个月,两组的NRS评分均较治疗前有显著性差异(P<0.01);治疗第3、5、7天,两组间NRS评分比较有统计学差异(P<0.05),且B组有效率高于A组(P<0.05)。结论:硬膜外PCA输注吗啡可快速、有效缓解腹部带状疱疹后神经痛,显著降低NRS评分,提高治疗效果。Objective：To investigate the effect of patient controlled analgesia（PCA）with epidural injection of morphine on the numerical rating scales（NRS）of postherpetic neuralgia.Methods：Thirtyeight patients with abdominal herpes zoster neuralgia were randomly divided into two groups：group A（n=16）and B（n=22）.An epidural catheter was inserted into the intervertebral space between T12 and L1 vertebra for all patients,and the catheter was connected with a PCA pump containing complex analgesics,with morphine in group B and without morphine in group A.The numerical rating scales（NRS）were recorded for pain evaluation.Results：There was significant difference in NRS between before treatment and 3,5,7days after treatment and 30 days after discharge from hospital in both groups（P〈0.01）.There were obvious differences in NRS 3,5,7days after treatment between group A and B（P〈0.05）,and the patients in group B obtained better analgesia results（P〈0.05）.Conclusion：The PCA with epidural injection of morphine can quickly and effectively relieve the pain caused by abdominal aherpes zoster,significantly decrease NRS and improve treatment effects.
